The promulgation and implementation of the two-child policy has prompted the emergence of a large number of two-child families.Existing studies have pointed out that the birth of the second child will lead to changes in family relations and family structure, and some of the eldest children in the two-child family have entered the stage of adolescence. The age gap,the particularity of adolescents’ psychology and the arrival of the second child cannot be ignored. The impact of the eldest child’s new role identity, the lack of parental support, the weak sense of belonging in school and the difference of social culture and concept are the important reasons for the complication of family relationship, which will undoubtedly bring unique challenges to the psychological growth of the eldest child in adolescence.Based on rational emotion therapy,Erikson’s personal identity theory and Satir’s family therapy model, this paper analyzes, summarizes and solves the factors affecting the achievement of personal identity of older children from individual level and family level respectively by using case work method. At the personal level, the personal emotion iceberg map is used to strengthen the client’s selfunderstanding and correct his irrational beliefs through rational emotion therapy. At the family level, family sculpture, meditation and other techniques are used to improve the communication mode between members and rebuild family rules. The case work method in this study effectively urged the subjects to coordinate family relations,be close to parents, fraternize with brothers, and successfully achieve personal identity. This provides a useful reference for the social work intervention model aimed at such clients to help the adolescent children solve family conflicts, improve their sense of personal identity and form personal identity.
