Application Of Case Management In Long-term Care Of Disabled Elderly At Home

China’s aging is becoming more and more serious.According to the latest data released by the National Bureau of statistics,the elderly in China have an average of about 8 years of living with diseases,and some elderly people are disabled due to the gradual degradation of their physical functions,further reducing their quality of life.Influenced by China’s traditional filial piety culture and the view of providing for the aged,the elderly in China,especially the disabled elderly,are still more inclined to choose the home-based elderly care model at this stage.Therefore,how to meet the long-term care needs of this part of the disabled elderly has become a problem that can not be ignored in the cause of providing for the aged in China.Through reading and analyzing the relevant literature on the long-term care of the home-based disabled elderly,the author found that the relevant literature focuses on analyzing the needs,existing problems,policy choices and possible development models of the long-term care of the homebased disabled elderly in China at this stage,but lacks the exploration on how to provide longterm care for the home-based disabled elderly.Case management is a comprehensive service method,which emphasizes that social workers link and integrate relevant resources for service objects in the service,aiming to provide comprehensive services for service objects with multilevel service needs and meet the needs of objects.At the same time,the case management model also has certain stability and durability,and can provide long-term care services for service objects with long-term care service needs.Therefore,applying the case management method to the long-term care of the home-based disabled elderly can not only meet the long-term care needs of the home-based disabled elderly,but also provide lasting and long-term care services for the home-based disabled elderly by establishing a social support network.This paper is based on the author’s internship experience in Guangzhou x institution.First of all,a random sample of 30 home-based disabled elderly in X institutions in Guangzhou was conducted,and a typical case was selected as the object of this paper.Secondly,the process of applying case management to the long-term care of the disabled elderly at home is explored in the way of field research.At the end of the service,the intervention effect is evaluated according to the unstructured interview method and unstructured observation method,so as to test the effectiveness of the case management method.After the service,the author discussed and analyzed the whole service process,pointed out the problems and reasons in the application of case management in the long-term care of the disabled at home,and tried to put forward measures to solve these problems,so as to provide practical reference for the application of case management in the long-term care of the disabled at home in the future.Through the case management service process,the author found that the application of case management in the long-term care of the home-based disabled elderly can bring significant benefits to the home-based disabled elderly.For example,enrich the formal social support network of service objects,and make up for the informal social support network of service objects.However,there are still some problems in the case management service,such as the solidification of the service provider,which is difficult to meet the dynamic needs of the service object;The integration of various services is not high,and the service quality is difficult to guarantee;The service focus is not clear,and the service characteristics of case management are not highlighted.The reasons for the above problems are the lack of the role of social work service manager,the rigidity of service evaluation mechanism and the lack of communication and coordination among service subjects.Through the analysis of things,the author believes that the above problems can be solved from the following aspects:strengthening the supervision of the support of social workers,realizing the dynamic management of the service process,and unblocking the communication and coordination among the service subjects.Through the case management service in this paper,the author intends to expand the practical scope of case management and enrich the methods of providing long-term care for the disabled elderly at home.
