Purpose: To compare the clinical efficacy of worm medicine combined with Zhi Kidney and Blood Stasis Tang treatment with that of Zhi Kidney and Blood Stasis Tang treatment alone in patients with perimenopausal headache due to kidney deficiency and blood stasis. This study was conducted to verify that the treatment of perimenopausal headache with kidney deficiency and blood stasis is more effective based on the theory of Loxoplasmosis, and to provide new ideas for the clinical treatment of perimenopausal headache in the field of gynecology.Material and method: The 66 patients who met the inclusion criteria for this clinical observation were patients with perimenopausal headache with kidney deficiency and blood stasis evidence who attended the gynecology outpatient clinic of the Affiliated Hospital of Liaoning University of Chinese Medicine from August 1,2021 to December 1,2022.Based on the random number table method, all patients were divided into a test group and a control group.Thirty-three cases in each group were given to the control group(zhi kidney and blood stasis soup)orally and the experimental group(worm medicine + zhi kidney and blood stasis soup orally)to observe the efficacy. The single treatment cycle was 1 month, and 2 courses of treatment were given to observe the changes in VAS score, total headache score by scoring method,TCM evidence score, quality of survival scale, and serum sex hormone levels(including FSH,LH,E2,AMH)before and after treatment in both groups.Results:1.VAS scores decreased after treatment in both the test and control groups of patients, with a greater decrease in the test group, which was considered significantly different between the test and control groups(P < 0.05).By comparing the overall efficacy of VAS scores,in the test group,3 patients were cured,14 patients had significant effect,11 patients were effective, and 2 patients were ineffective, with a total effective rate of 93%;in the control group,1 patient was cured,10 patients had significant effect,11 patients were effective, and 8 patients were ineffective, with a total effective rate of 73%.Compared with the control group, the efficacy of the experimental group was better, and there was a significant difference between the two(P < 0.05).2.The degree and duration of headache decreased after treatment in both the test and control groups, with a greater decrease in the test group, which was considered a significant difference between the test and control groups(P < 0.05).Although the number of headache attacks decreased and the accompanying symptoms improved in both groups, there was no significant difference between them(P > 0.05).3.The total scored headache symptom scores decreased after treatment in both the test and control groups, with a greater decrease in the test group, which was considered a significant difference between the test and control groups(P < 0.05).By comparing the overall efficacy of headache symptom score,in the test group,4 patients were cured,8 patients had significant effect,14 patients were effective, and 4 patients were ineffective, with a total effective rate of 87%;in the control group,2 patients were cured,6 patients had significant effect,11 patients were effective, and 11 patients were ineffective, with a total effective rate of 63%.Compared with the control group, the efficacy of the test group was better, and there was a significant difference between them(P < 0.05).4.The TCM symptoms scores of patients in both the test group and the control group decreased after treatment, with a greater decrease in the test group, which was considered a significant difference between the test group and the control group(P < 0.05).By comparing the overall efficacy of the TCM symptoms,in the test group,5 patients were cured,9 patients had significant effect,13 patients were effective, and 3 patients were ineffective, with a total effective rate of 90%;in the control group,1 patient was cured,5patients had significant effect,14 patients were effective, and 10 patients were ineffective, with a total effective rate of 67%.Compared with the control group, the efficacy of the experimental group was better, and there was a significant difference between the two(P<0.05).5.The quality of survival scale scores increased in both the test and control groups after treatment, with a greater increase in the test group, and a significant difference was found between the test and control groups(P<0.05),and the patients in the test group had better quality of survival after treatment.Conclusion:1.The combination of worm medicine with Zhi Kidney and Blood Stasis Tang for the treatment of perimenopausal headache with kidney deficiency and blood stasis evidence can significantly reduce the pain level of patients, and the efficacy is better than that of only taking Zhi Kidney and Blood Stasis Tang.2.The combination of worm medicine and Zhi Kidney and Blood Stasis Tang for the treatment of perimenopausal headache with kidney deficiency and blood stasis can reduce the duration of headache and is more effective than taking only Zhi Kidney and Blood Stasis Tang.3.Both groups were able to reduce the number of headache attacks and concomitant symptoms, but there was no significant difference in the efficacy.4.Both groups were able to improve the quality of survival scores of patients, and the efficacy of the combination of worm medicine with Zhi kidney and expelling blood stasis soup was better. In conclusion, both the experimental group and the control group were able to achieve better therapeutic effects on perimenopausal headache with kidney deficiency and blood stasis. Both groups were able to reduce the intensity, number and duration of pain during headache attacks and reduce concomitant symptoms. The overall efficiency of the combination of worm medicine and Zhi Kidney and Blood Stasis Tang was higher than that of oral medicine only, indicating that the overall effect of the combination of worm medicine and Zhi Kidney and Blood Stasis Tang in the treatment of perimenopausal headache is more obvious and efficient based on the theory of Loxodistrophy.
