Exploring The Rules Of Traditional Chinese Medicine For Vitiligo Treatment Based On Data Mining And Summarizing The Clinical Experience Of Tutor

Purpose:Based on the data mining technology to explore the law of medicine treatment of vitiligo in traditional Chinese medicine;To summarize the experience of Professor Liu Bin’s clinical treatment of vitiligo;In order to provide a new idea for the clinical treatment of vitiligo with traditional Chinese medicine,the drug use rules of the two were analyzed and compared.Material and method:Study 1:In the database of China National Knowledge Network(CNKI),Wanfang database and Weip.com database,the keywords"vitiligo","Bai Feng","Bai Ge Feng"and"purpura"were searched.According to the inclusion and exclusion criteria,the literature on the internal treatment of vitiligo by traditional Chinese medicine was screened,the names of traditional Chinese medicine were standardized and the database was constructed.Data mining was conducted using v2.5,an auxiliary platform for traditional Chinese medicine inheritance,and data analysis methods such as syndrome statistics,drug frequency statistics,sex and taste regression statistics,association rules and cluster analysis were used to analyze the medication rules of traditional Chinese medicine for vitiligo.Study 2:Vitiligo cases were collected from the outpatient clinic of the Department of Dermatology of the Affiliated Hospital of Liaoning University of Chinese Medicine from December 2020 to December 2022,and 79 cases that met the criteria were screened. Establish an Excel table to record patient information in detail,including patient case number,gender,age,lesion location,lesion area,TCM symptoms,treatment and prognosis.SPSS25.0 was used for statistical analysis of efficacy data.And the same method as the above is used to summarize the medication rules,and the two medication rules are analyzed and compared.Results:Study 1(Documentary Medical Case):1.TCM syndrome differentiation:101 prescriptions involved five TCM syndrome types,including deficiency of liver and kidney(35.64%),discordation of qi and blood(23.76%),blockage of meridians and collaterals(16.83%),qi stagnation of liver(15.84%)and weakness of spleen and stomach(7.93%).2.Drug classification:A total of 209 drugs were used in this study,involving 16 drug classifications.The classification of high-frequency drugs from high to low was as follows:tonifying deficiency drugs(48 drugs),clearing heat drugs(26 drugs),relieving surface drugs(22 drugs),promoting blood circulation and removing blood stasis drugs(20 drugs).3.Frequency of menstrual normalization of sexual flavor:The frequency of medicinal properties of 209 TCM from high to low was:warm(638),cold(416),flat(283),cool(63),hot(12).The frequency of medicinal taste from high to low was:sweet(749),bitter(643),octinous(636),sour(126),salty(50),astringent(47).The frequency of meridian conversion:liver(874),spleen(582),kidney(463),heart(430),lung(389),stomach(307),large intestine(153),gallbladder(139),bladder(126),pericardium(44),small intestine (40),Sanjiao(22).4.High-frequency drugs of different syndrome types:the frequency of high-frequency drugs used in syndrome of liver and kidney insufficiency was psoralea,Tribulus terrestris,Fructus ligustris,Grasscourt and angelica sinensis in descending order.The frequency of high frequency drug use in Qi and blood disharmony syndrome was angelica sinensis, Tribulus terrestris,parsnips,Astragalus membranaceus and ligusticum Chuanxiong from high to low.The frequency of high frequency drug use in meridians stasis syndrome was safflower,angelica sinensis,salvia miltiorrhiza,peach kernel and astragalus.The frequency of high frequency drug use in liver qi stagnation syndrome from high to low was angelica sinensis,bupleurum bupleurum,Paeony,parsnips and salvia miltiorrhiza. The frequency of high frequency drug use in spleen and stomach weakness syndrome was pyrohawthorn and poria from high to low.5.The analysis of drug association rules showed that the five most commonly used drug pairs were psoralea and Tribulus terrestris,psoralea and angelica sinensis,angelica and Tribulus terrestris,xerolea and Ligustrum,and salvia miltiorrhiza and Tribulus terrestris,which involved a total of 6 TCM.6.Cluster analysis revealed five potential new prescriptions of core drugs:"Wei Ling Hsien＿Corydalis＿Xinyi＿Silkworm cocoon","Scorched six Shenqu＿Scorched malt＿processed Astragalus＿light bamboo leaves","Atractylode＿Sophora flavescens＿heavy building＿Solitary life","Teristles Teristles＿Ligustrine＿Danshen＿psoralea＿Baizhi＿mulberry white bark",and Fructus aurantii＿Yu Jin＿Xiangfu＿Cassia branch".Study 2(Tutor’s Medical Case):1.Treatment results:the skin lesion area of the patient before treatment was 9.08±5.09cm~2;The lesion area at week 12 was 5.76±4.94 cm~2;The area of lesions before treatment and the area of lesions at week 12 was significantly reduced(P<0.05,a statistically significant difference).Clinical efficacy of Western medicine:The total effective rate of clinical efficacy of Western medicine in the 12th week of treatment was89.87%.The scores of TCM syndrome in the 12th week of treatment were significantly lower than those before treatment(P<0.05,the difference was statistically significant).Efficacy of TCM symptoms:The total effective rate of TCM symptoms in the 12th week of treatment was91.14%;The total effective rates of different types are:qi and blood discord evidence effective rate85.71%,liver depression and qi stagnation evidence, effective rate80%,spleen and stomach weakness and effective rate100%,meridian stasis obstruction evidence,effective66.67%liver and kidney insufficiency evidence, effective93.65%.2.TCM differentiation classification:79 prescriptions involved five TCM syndrome types,liver and kidney insufficiency(79.75%),qi and blood discord(8.86%),liver depression and Qi stasis(6.33%),meridian stasis(3.80%),spleen and stomach weakness(1.26%).3.Drug frequency of liver and kidney deficiency:63 prescriptions involved 42 drugs,and the frequency of more than 10 times were Angelica,Dangshen,Beisha,Salvia miltiorrhiza,white skin,psorion,privet,mulberry,teristus,sinensis,blood vine,bupleurum,Scutellaria baicalensis,cassia branch,summer grass,fragrant,and black sesame.The frequency was20 times or more,including angelica,Dangshen,Beisha shen,Salvia miltiorrhiza,Salvia miltiorrhiza,white skin,psorion,virgin,mulberry,teristus.4.Drug classification:A total of 42 drugs were used in this study,involving 9 kinds of drugs.The high frequency drugs were deficiency drug(15),heat clearing drug(11), solution drug(5),and blood circulation and blood stasis drug(3).5.Sexual Taste Attributed Frequency:The frequency of 42 flavors of Chinese medicine medicinal properties is,from high to low:cold(254),warm(152),flat(95),cool(27).The frequency of medicinal taste from high to low is:bitter(323),sweet(311),bitter(171),sour(33),salty(8),astringent(8).Frequency of menstruation:liver meridian(293),spleen meridian(233),lung meridian(168),heart meridian(156),kidney meridian(128), stomach meridian(127),pericardial meridian(64),bladder meridian(52),biliary meridian(44),large intestine meridian(31),small intestine meridian(13),trifocal meridian(10).6.Drug association rule analysis,the most commonly used 11 groups of drug pairs are party participation angelica,party participation in northern sand ginseng,angelica and north sand ginseng,party ginseng,angelica and north sand ginseng,dan participation in party ginseng,dan participation in angelica,dan participation in north sand ginseng, danshen,party participation in north sand ginseng,danshen,angelica and north sand ginseng,danshen,party ginseng,angelica and north sand ginseng,a total of 4 flavors of traditional Chinese medicine involved.7.Cluster analysis revealed five potential new prescriptions of core drugs,such as"Codonopsis Moutan bark-Atractylodes-Angelica sinensis-Phellodes Phellodes", "Codonopsis Terrestris＿Tribulus Terrestris＿white fresh skin＿Scutellaria","Tribulus terrestris＿Polygonum multiflorum＿Prunus prunus＿mulberry＿subspatholobi","Psorus terrestris＿mulberry＿Tribulus Terrestris＿white fresh skin＿subspatholobi＿cassia twig".Conclusion:1.In this study,the main syndrome types of vitiligo in medical documents were mainly syndrome of deficiency of liver and kidney and syndrome of disharmony between qi and blood.Drugs are classified as supplementing deficiency drugs,clearing heat drugs, relieving surface drugs and promoting blood circulation and removing blood stasis drugs.Warm,sweet and liver channels were the main sexual flavor.Multi-purpose deficiency medicine,supplemented by tonifying blood and relieving surface or calming liver and wind as medicine pair;The potential new prescription obtained through cluster analysis focuses on the method of tonifying the liver and kidney,invigorating the spleen and stomach,supplemented by regulating qi and blood,soothing the liver and relieving depression,which provides certain reference for clinical practice.2.Professor Liu Bin,the supervisor of this study,has a definite effect on the internal treatment of vitiligo in traditional Chinese medicine;The main symptom of vitiligo in clinical cases is mainly liver and kidney insufficiency;The classification of drugs with liver and kidney deficiency is mostly used to supplement deficiency drugs,antipyretic drugs,anti-epidemiological drugs,and blood stasis removal drugs;Cold,bitter taste,liver meridian as the main taste;Use more tonic drugs to match,and blood circulation and stasis removal drugs as a drug pair;The potential new formula obtained through cluster analysis revolves around the tonic method,supplemented by liver thinning,blood activation and heat removal,which provides a certain reference for clinical practice.3.The clinical and medical cases in this study are basically consistent with the syndrome type of vitiligo in the literature,and the syndrome of deficiency of liver and kidney is the main syndrome type.In the treatment,besides toning liver and kidney,the teacher also makes good use of cassia twig to harmonise the health and improve the clinical effect.
