Purpose: Meta analysis was used to evaluate the efficacy and safety of replenishing qi and nourishing yin in the treatment of type 2 diabetes mellitus,in order to provide evidence-based medicine basis for clinical treatment of the disease.Material and method:The inclusion criteria and retrieval strategies were established, and the randomized controlled trials of replenishing qi and nourishing yin in the treatment of type 2 diabetes mellitus were collected by searching China knowledge Network,Wanfang Database,VIP Network,China Biomedical Literature Database,Pub Med,Embase and Cochrane Library. The retrieval time is from the establishment of the above database to December 2022.The retrieval literature titles were imported into the Note Express software, and the articles that met the criteria were screened and re-screened according to the inclusion criteria and exclusion criteria, and the biased risk assessment of the literature was carried out in the Revman5.4 software according to the Cochrane risk bias assessment tool.finally, the relevant data of the outcome indicators were input into the Revman5.4 software analysis, and the results of this Meta analysis were obtained.Results:1 Meta-analysis results1.1 A total of 764 articles were obtained, and 24 articles were included after screening. The treatment group was treated with the method of replenishing qi and nourishing yin on the basis of the control group, and the control group was treated with routine western medicine alone.A total of 1774 patients, including the treatment group(n = 857)and the control group(n = 917).1.2 Total effective rate: the treatment group could improve the overall clinical effect [OR and95%Cl 4.13,10.22(P<0.00001)].1.3 Fasting blood glucose(FPG): the effect on FPG in the treatment group was significantly different from that in the control group [MD and 95%Cl were-0.98(-1.28,0.67),Zhuan 6.20(P<0.00001)],the difference was statistically significant.1.4 2-hour postprandial blood glucose(2h PG): the effect of improving 2-hour postprandial blood glucose in the treatment group was significantly different from that in the control group[MD and 95%Cl were-1.35,1.72,0.97].C1.5 Glycosylated hemoglobin(Hb A1c): the improvement of Hb1 Ac in the treatment group was significantly different from that in the control group [MD and 95%Cl were-0.80,1.17,0.44 and4.32,respectively].1.6 Total cholesterol(TC): the improvement of TC in the treatment group was significantly different from that in the control group [MD and 95%Cl were-0.57(-1.02,-0.12),Zhuan 2.50(P<0.01)],the difference was statistically significant.1.7 Triglyceride(TG): the improvement of TG in the treatment group was significantly different from that in the control group [MD and 95%Cl were-0.35,0.51,-0.19,4.24 and 0.0001],and the difference was statistically significant.2 Analysis of drugs, four sexes, five flavors, meridians and drug types:A total of 24 literatures were included for this study, including 59 Chinese herbs.Frequency analysis of traditional Chinese medicine: the top 5 in the order of frequency of use are Astragalus,Rehmannia glutinosa,Snakegourd Root,Salvia,Yam,and Pueraria.Analysis of four qi and five flavors: the cold drugs were used the most with a frequency of 106 times;the sweet drugs were used the most with a f requency of 185 times.Analysis of drug meridians:The meridians of the drugs from high to low frequency were lung meridian, spleen meridian, kidney meridian, and stomach meridian.Analysis of drug efficacy: the top 5 in the order of frequency of use are tonic medicine,heat-clearing medicine, blood circulation promoting and stasis removing medicine,exterior-releasing medicine, and astringent medicine.Analysis of association rules of traditional Chinese medicines: The top 3 drug pairs used are "Astragalus-Rehmannia glutinosa","Astragalus-Snakegourd Root","Astragalus-Yam",and the core drugs are Astragalus,Rehmannia glutinosa,Salvia,Snakegourd Root,Pueraria,Yam,and Dwarf Lilyturf.Conclusion:1.The Meta analysis shows that the method of replenishing qi and nourishing yin combined with western medicine in the treatment of type 2 diabetes mellitus is superior to simple western medicine in improving the total clinical effective rate, reducing the related indexes of glucose metabolism and lipid metabolism, and the treatment group has less adverse reactions and higher safety.2.According to the results of drug frequency studies,it can be concluded that Poria cocos is the most commonly used drug in the treatment of type 2 diabetes mellitus, and the other more commonly used traditional Chinese medicines are Radix Astragali,Radix Rehmanniae,Radix Trichosanthis,Salvia miltiorrhiza, yam, and Pueraria lobata.3.The most common drugs used by traditional Chinese medicine to improve the inflammatory response of T2 DM are cold and sweet, and the main channels are the lung, spleen, and kidney channels. The core drugs for improving the inflammatory response of T2 DM are huangqi, shengdihuang, and tianhuafen.
