Clinical Observation On The Therapeutic Effect Of Ziyin Tongbian Prescription Combined With Yixiao Ointment In The Treatment Of Ungut Hemorrhoids Of Blood-heat And Intestinal Dryness Typ

Purpose:To observe the clinical effect of Ziyin Tongbian prescription combined with Yixiao ointment and hemorrhoids powder on the treatment of anal fissure with blood-heat and intestinal dryness.Material and method:In the method of clinical efficacy observation,114 patients with uncinate intestinal hemorrhoids who met the standard of sodium drainage were selected from the anorectal outpatient department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 2022 to December 2022.The basic data and disease information of the patients were counted at the initial diagnosis,and they were randomly divided into 3 groups according to the random number table method,namely group A,group B and group C,with 38 cases in each group.The intervention methods of group A were oral Ziyin Tongbian prescription plus hemorrhoids loose fumigation;the intervention methods of group B were external application of Yixiao ointment plus hemorrhoids loose fumigation;the intervention methods of group C were oral Ziyin Tongbian prescription,external application of Yixiao ointment and hemorrhoids loose fumigation.Patients’ progress was tracked and recorded during treatment.The improvement of constipation,pain,blood in stool,cleft healing and fear of defecation in the three groups were observed and compared before treatment and at the 3rd,7th and 14 th day of treatment.The observation of constipation included stool frequency,defecation time and fecal traits Bristol scale score.The pain relief was compared from VAS score and pain duration.Observe the improvement of PSQI score for sleep disorder and the changes of tongue and pulse in three groups before and after treatment.Data were sorted by EXCEL and analyzed statistically by SPSS Statistics 25.0software.Results:1.A total of 114 patients were included in this study,including 17 cases of shedding,32 cases in the final group A,33 cases in the group B,and 32 cases in the group C.There were no significant differences in gender,age,and course of disease among the three groups,P >0.05,indicating comparability.2.The frequency of stool on the 14 th day of the three groups was statistically significant compared with that before treatment,P < 0.05;On the 14 th day of treatment,there was a significant difference between the group C and the group B(P < 0.01),but no difference between the group C and the group A(P > 0.05).3.Defecation time,Bristol score and fear of defecation on the 14 th day of the three groups were significantly different from those before treatment,P < 0.01;On the 14 th day of treatment,the comparison of defecation time,Bristol score and fear of defecation among groups showed statistical significance between the group C and the group B(P < 0.05),while there was no statistical significance between the group A and the other two groups(P > 0.05).4.VAS scores and pain duration of the three groups on day 14 were statistically significant compared with those before treatment,P < 0.01;On day 14 of treatment,VAS scores were significantly different between the group C and the other two groups(P < 0.01),and pain duration was significantly different between the group C and the group A(P < 0.05).5.There were significant differences between the three groups on the 14 th day in hematostocia score and gap healing compared with before treatment,P < 0.01;On the 14 th day of treatment,there were differences between the group C and the other two groups in stool blood score and tear healing,P < 0.05.6.The sleep disorder on the 14 th day of the three groups was significantly different from that before treatment,P < 0.01;On day 14,there was a difference between the group C and the group A(P < 0.05).7.The effective rate of the group A was 90.6%,that of the group B was42.4%,and that of the group C was 100%.There was a significant difference between the group C and the group B(P < 0.01),and that of the group A(P > 0.05).The difference is not obvious.8.The total effective rate of the group A was 78.1%,that of the group B was 78.8%,and that of the group C was 96.9%.The comparison between the group C and the other two groups had statistical significance(P < 0.05).Conclusion:1.The three treatment methods can achieve significant clinical efficacy in the treatment of constipation,pain,blood in the stool,tear healing,fear of defecation,and sleep disorders.2.Oral Ziyin Tongbian prescription combined with external application of Yixiao ointment and hemorrhoid powder fumigation had better efficacy than external application of Yixiao ointment combined with hemorrhoid powder fumigation in reducing stool frequency,shortening defecation time,changing stool characteristics,relieving pain,reducing blood in stool,promoting tear healing and reducing patients’ fear of defecation.3.Oral Ziyin Tongbian prescription combined with external application of Yixiao ointment and hemorrhoid powder fumigation had better efficacy than oral Ziyin Tongbian prescription combined with hemorrhoid powder fumigation in shortening defecation time,relieving pain,shortening pain duration,reducing blood in stool,promoting tear healing and improving sleep disorders.4.The clinical effect of oral Ziyin Tongbian prescription combined with external application of Yixiao ointment and hemorrhoid powder fumigation in the treatment of anal fissure of blood-heat and intestinal dryness type is better than that of simple oral Ziyin Tongbian prescription combined with hemorrhoid powder fumigation or simple Yixiao ointment combined with hemorrhoid powder fumigation.
