Objective: To observe the clinical effect of Qingyi decoction combined with Tibetan medicine Phyllanthus emblica in the treatment of mild acute pancreatitis(liver depression and qi stagnation)Material and method:TCMSP(Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform)database was used to obtain the main active components and main targets of Phyllanthus emblica.The disease targets of mild acute pancreatitis were obtained through Gencards database.The Venny platform was used to obtain the common targets of Phyllanthus emblica and mild acute pancreatitis.PPI protein data network was constructed in String database.GO and KEGG analyses were performed using the Metascape platform.Cytoscape 3.7.2 software was used to construct the "target-pathway" network and analyze the important pathways.Finally,the most important target for the treatment of mild acute pancreatitis is obtained.According to the inclusion and exclusion criteria,40 patients with mild acute pancreatitis(liver depression and qi stagnation)admitted to the Department of General Surgery,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were selected.They were randomly divided into observation group and control group,with 20 cases in each group.The control group was treated with conventional western medicine and Qingyi decoction orally(3 times/day,100ml/ time),and the observation group was treated with 18 g Phyllanthus emblica added to Qingyi decoction orally(3 times/day,100ml/ time)on the basis of the control group.The observation course was 10 days.The curative effect,relief of abdominal pain and distension,recovery of exhaust and bowel sounds,recovery of pancreatic enzymes,recovery of white blood cells and C-reactive protein were observed in the two groups.The observation index data were recorded in detail and analyzed by SPSS 26.0 software.Results:1.Network pharmacology showed that the main active components of Emba in the treatment of mild acute pancreatitis were ellagic acid,catechin,kaempferol,quercetin;The key targets included MAPK1,RELA,AKT1,JUN,TNF and IL6."The main biological processes are response to inorganic substances,positive regulation of locomotion,response to hormones,and response to bacterial origin.The reaction of molecules,reactive oxygen species;It mainly acts on protein kinase activity and protein homodimerization activity sex,phosphotransferase activity,alcohol-based receptor,DNA transcription factor binding,protein serine/threonine / Molecular functions such as tyrosine kinase activity;The main enriched pathways included cancer pathways and diabetic complications AGE-RAGE signaling pathway,PI3K-Akt signaling pathway,proteoglycan in cancer,tumor necrosis factor signaling signal transduction pathway,MAPK signaling pathway,etc.2.Clinical observation results showed that:2.1 Total effective rate of acute pancreatitis: the observation group was higher than the control group.The AP of the observation group was significantly higher than that of the control group p(27)0.05.2.2 After treatment,the scores of abdominal pain and abdominal distension in the observation group were significantly lower than those in the observation group,p(27)0.05.The observation group was better than the control group.The symptoms of abdominal pain and bloating were improved.2.3.In terms of the recovery time of serum amylase,urine amylase,lipase,white blood cells,and C-reactive protein.The observation group was significantly faster than the control group,p(27)0.05.Compared with the control group,the observation group had significantly shortened pancreatic enzymes and inflammatory indicators recovery time.2.4 The recovery time of exhaust and bowel sounds in the observation group was significantly faster than that in the control group(p(27)0.05).The observation group was better than the control group.The recovery time of exhaust and bowel sounds was significantly shortened.2.5 The development rate of severe cases,the development rate of severe cases in the observation group was significantly lower than that in the control group,in reducing the development rate of severe cases.The experimental group was significantly better than the control group,p(27)0.05.Conclusion:1.Qingyi Decoction combined with Phyllanthus emblica can significantly improve the clinical efficacy of MAP treatment.2.Network pharmacological studies have initially revealed that the treatment of mild acute pancreatitis by phyllanthus embalum is the result of multi-component,multi-target and multi-pathway interaction,among which the most important one is the inflammator-related target pathway.3.Qingyi Decoction combined with Phyllanthus emblica can significantly improve the symptoms of abdominal pain and abdominal distension in MAP patients;Accelerate the recovery time of exhaust,bowel sound,shorten the recovery time of physical signs,pancreatic enzymes and inflammatory indicators;It can reduce the development rate of severe diseases and is worthy of clinical application.
