| Objectives:To investigate the current status of frailty in elderly patients with stroke,and to explore the correlation between frailty and anxiety,depression,and social support in elderly patients with stroke,and the influencing factors of frailty in elderly patients with stroke.Methods:Using cluster sampling,from June 2021 to January 2022,elderly patients with stroke(stroke)who were hospitalized in the neurology department(or encephalopathy department)and rehabilitation department of five tertiary general hospitals in Changsha City,Hunan Province were selected as the research objects.The survey tools were self-designed general situation questionnaire,Chinese version of Tilburg Frailty Scale,Generalized Anxiety Disorder Scale,Patient Health Questionnaire Depression Scale,and Social Support Rating Scale.Statistical analysis was carried out on the final collected data.The general conditions of elderly stroke patients were described by frequency and percentage;the scores of frailty,anxiety,depression and social support of elderly stroke patients with different characteristics were subject to normal distribution and homogeneity of variance,and independent sample t was used.Test or analysis of variance,non-parametric test(Mann-Whitney U test or Kruskal-Wallis H test)if it does not obey the normal distribution or unequal variance;Spearman correlation analysis is used to explore the relationship between frailty and anxiety,depression,anxiety and depression in elderly stroke patients.Correlation of social support;multiple linear regression analysis was used to explore the influencing factors of frailty in elderly stroke patients.Results:(1)A total of 505 subjects were included in this study.The prevalence of frailty in elderly stroke patients was 86.9%;the prevalence of anxiety was 75.2%;the prevalence of depression was 82.7%;Among them,79.0% were at the middle level,and 21% were at the high level.(2)The total frailty score of elderly stroke patients is 8.15 points,of which the physical frailty dimension score is 4.24 points,the psychological frailty dimension score is 2.74 points,and the social frailty dimension score is 1.17 points;the total score of anxiety is 8.38 points;the total score of depression is 8.38 points.9.64 points;the total score of social support is 39.39 points,of which the score of objective support is 10.54 points,the score of subjective support is 21.55 points,and the score of social support utilization is 7.3 points.(3)The total frailty score of elderly stroke patients in different age,BMI value,alcohol consumption,self-care ability,primary caregiver,sleep time,condition at onset,symptoms at onset,stroke classification,incidence of cerebral infarction,incidence of cerebral hemorrhage,swallowing disorder,sequelae,hemiplegia site,fasting blood glucose,hemoglobin,serum uric acid,fibrinogen,D-dimer were significantly different(P<0.05)(4)Total anxiety score of elderly stroke patients in different self-care ability,main caregiver,sleep time,condition at onset,course of disease,dysphagia,TCM rehabilitation treatment,sequelae,hemiplegia,hemoglobin,fibrinogen,prothrombin time,D-dimer,there were significant differences(P<0.05).(5)The total score of depression in elderly stroke patients varies in age,BMI,educational level,living environment,smoking,drinking,self-care ability,primary caregiver,per capita monthly family income,sleep time,condition at onset,family history,TCM rehabilitation,There were significant differences in sequelae,dysphagia,hemiplegia site,fasting blood glucose,low density lipoprotein,creatinine,fibrinogen,prothrombin time and D-dimer(P<0.05).(6)The total score of social support in elderly patients with stroke is age,education level,marital status,living environment,living style,drinking,smoking,exercise habits,self-care ability,per capita monthly household income,condition at onset,course of disease,stroke classification,and occurrence of cerebral infarction There were significant differences in frequency,dysphagia,family history,TCM rehabilitation treatment,sequelae,platelets,serum albumin and D-dimer(P<0.05).(7)The frailty of elderly stroke patients was positively correlated with anxiety level and depression level(P<0.05),and negatively correlated with social support level(P<0.05);multiple linear regression analysis showed the influence of frailty in elderly stroke patients The factors were age,self-care ability,depression level,anxiety level and social support level.Conclusions:(1)Elderly stroke patients have a higher incidence of frailty.(2)Anxiety,depression and frailty were positively correlated in elderly stroke patients,while social support level was negatively correlated with frailty.(3)Age,self-care ability,depression,anxiety and social support level of elderly stroke patients are the main influencing factors of frailty in elderly stroke patients. |
