Clinical Observation On The Treatment Of Chronic Heart Failure With Diuretic Resistance Due To Yang Deficiency, Blood Stasis And Water Retention With Shenfu Wenyang Huoxue Decoctio

Objective: By observing the clinical efficacy and safety of Shenfu Wenyang Huoxue Decoction in the treatment of yang deficiency,blood stasis and water stop syndrome in patients with chronic heart failure and diuretic resistance,it will provide a basis for further clinical application.Methods: According to the random number table method,60 patients with chronic heart failure with diuretic resistance who met the inclusion criteria were divided into the control group and the experimental group with 30 cases in each group.The control group was given routine treatment,and the experimental group was additionally given Shenfu Wenyang Huoxue Decoction,twice a day,for 14 days in both groups.The efficacy and scores of TCM syndromes,the efficacy of heart failure(NT-pro brain natriuretic peptide(NT-pro BNP),the New York Heart Association(NYHA)of the two groups of patients before and after treatment were observed before and after treatment Cardiac function classification,Lee’s score for heart failure,Left Ventricular Ejection Fraction(LVEF)),efficacy of diuretic resistance(24h urine output,fasting body weight),and drug safety evaluation.Results:1.CHF efficacy: There was no significant difference in NT-pro BNP,NYHA cardiac function class,Lee’s heart failure score and LVEF between the two groups before treatment(P>0.05).(1)NT-pro BNP: after treatment,both groups of NT-pro BNP were compared with those before treatment,P < 0.01,and the difference was statistically significant;after treatment,compared with P < 0.05,the difference was statistically significant.In the control group;(2)NYHA cardiac function classification: after treatment,the total effective rate of the control group(18/30)was 60.00%;after treatment,the total effective rate of the experimental group(24/30)was 80.00%;Compared with the two groups,P<0.05,the difference is statistically significant;(3)Lee’s heart failure score: after treatment,the total effective rate of the control group(20/30)was 66.67%;after treatment,the total effective rate of the experimental group(25/30)was 83.33%;Compared with the two groups,P<0.05,the difference is statistically significant;(4)LVEF: After treatment,the LVEF between the two groups was P<0.05 compared with that before treatment,and the difference was statistically significant;after treatment,the LVEF between the two groups was compared with P<0.01,the difference was statistically significant,and the treatment group was more significant.2.Efficacy of diuretic resistance: there was no significant difference in 24 h urine volume and fasting body weight between the two groups before treatment(P>0.05);(1)24h urine volume: after treatment,compared with P<0.01,the difference was statistically significant,The treatment of the experimental group was more significant than that of the control group;(2)Fasting body weight: after treatment,the difference was statistically significant between the two groups,P<0.01,and the therapeutic effect of the experimental group was better than that of the control group.3.Efficacy of TCM syndromes: after treatment,the total effective rate of the control group(25/30)was 83.33%;after treatment,the total effective rate of the experimental group(28/30)was 93.33%;after treatment,the comparison between the two groups was P<0.05,the difference was statistically significant significance.4.TCM syndrome scores: there was no significant difference in TCM syndrome scores between the two groups before treatment(P>0.05);there was a statistically significant difference between the two groups before and after treatment(P<0.01);after treatment,the difference between the two groups was P <0.01,the difference is statistically significant,which proves that the treatment of the experimental group is more significant.5.Evaluation of drug safety: There were few adverse reactions in the two groups during the clinical observation period,which confirmed that the clinical application of Shenfu Wenyang Huoxue Decoction is safe and effective.Conclusion: Shenfu Wenyang Huoxue Decoction can significantly improve TCM syndromes,Lee’s heart failure score,improve cardiac function,LVEF,etc.,increase24 h urine volume,reduce NT-pro BNP level,fasting body weight,TCM syndrome scores etc.in patients with CHF and DR.It has clinical curative effect,and the clinical medication safety is good,without obvious adverse reactions and toxic side effects.
