| Objective:This project aims to observe the clinical efficacy of Dingyin decoction combined with conventional western medicine in the treatment of chronic heart failure with generalized Yang deficiency and water flooding,and to provide new ideas and directions for the treatment of chronic heart failure.Methods:In this study,a total of 76 patients with yang-deficiency and water-induced generalized chronic heart failure who were admitted to the Third Heart Disease Department of Shijiazhuang Hospital of Traditional Chinese Medicine from January 2022 to November 2022 were included and randomly divided into control group and treatment group with 38 patients in each group.The control group was given conventional Western medicine treatment according to the guidelines,and the treatment group was given traditional Chinese medicine Dingyin decoction on the basis of conventional Western medicine treatment.The two groups were treated continuously for 4 weeks,counted as 1 course.NT-pro BNP,LVEF,NYHA cardiac function grading,heart failure score(Lee’s score),TCM syndrome score and safety index were observed and compared between the two groups before and after treatment.Adverse events were recorded at the same time.The data obtained were analyzed and processed by SPSS 26.0software to evaluate the clinical efficacy.Results:(1)NT-pro BNP: After treatment,the NT-pro BNP value of both groups decreased(P<0.01),and the improvement effect of NT-pro BNP level in the treatment group was significantly better than that in the control group(P<0.01).(2)LVEF: LVEF in both groups was improved after treatment(P<0.01),and the degree of improvement was higher in the treatment group(P<0.01).(3)NYHA cardiac function grading: The cardiac function of both groups improved after treatment,with a response rate of 72.22% in the control group and 91.67% in the treatment group,indicating that the treatment group was better than the control group in improving cardiac function(P<0.05).(4)Heart failure score: Heart failure scores in both groups were decreased after treatment(P<0.01),and the decrease was more obvious in treatment group(P<0.05).The integral effective rate of heart failure in control group and treatment group were 58.33% and 86.11%,respectively,with statistical significance(P<0.01).(5)TCM syndrome score: After treatment,the total TCM syndrome score of the two groups decreased(P<0.01),and the treatment group was better than the control group(P<0.01).Compared with the two groups before and after the treatment,the control group had no statistical significance in the symptoms of chills,cold limbs,shortness of breath,fatigue and spontaneous sweating(P>0.05),while the other symptoms had statistical significance(P<0.05);After treatment,the score of TCM symptoms in the treatment group decreased significantly compared with that before treatment(P<0.05).There was no significant difference in the symptoms of palpitation,asthma,little urine and abdominal distension between the two groups(P>0.05),but there was significant difference in the improvement of other symptoms(P<0.05).The effective rate of the two groups was 72.22% in the control group and 94.44% in the treatment group,the difference was statistically significant(P<0.01).(6)Safety indicators: The safety indicators of patients were monitored before and after treatment in this study,and no abnormal changes were found,and no adverse reactions occurred during treatment.Conclusion: Dingyin decoction combined with conventional western medicine can not only improve the clinical symptoms and cardiac function classification of patients,but also improve the level of NT-pro BNP and LEVF,enhance myocardial contractility,and has good safety,which is worthy of promotion and use. |
