BackgroundSince the end of 2019,Corona Virus Disease 2019(COVID-19)has spread globally, and the Virus strain is generally susceptible in the population, among which middle-aged and elderly people and patients with chronic diseases are more likely to be infected. The mortality data of COVID-19 released by the Chinese Center for Disease Control and Prevention showed that the overall mortality rate after infection was 2.3%,and the mortality rate of patients with chronic non-communicable diseases was particularly high--10.5% of patients with cardiovascular diseases,7.3% of patients with diabetes,6.3% of patients with chronic respiratory diseases, and 6.0% of patients with hypertension.5.6% of patients with cancer.Outbreak of emergency and chronic disease prevention and control strategy for old patients bring huge challenges, which bring great changes to diabetics life, for the prevention and control of epidemic segregation, etc.,cut back on food type or uneven, outdoor activities of time frequency decreases,or bring patients with recent weight gain and broadening your waistline, insulin resistance increase,Blood Glucose is more difficult to control. In addition, patients may be afraid to enter the hospital under the epidemic situation, which may reduce the number of hospital visits, extend the interview time, change the drug purchase method or increase the incidence of drug leakage, reduce the drug dose independently, and emotional stress and anxiety, which may affect the Blood Glucose control rate of patients. This more increased the difficulty of diabetes Blood Glucose control and management, the new type of coronary pneumonia outbreak still need dynamic zero policy, belong to the prevention and control under the normal, during which put the emphasis on prevention and control of specific risk factors associated with diabetes, the diabetes and even chronic diseases as an important health hazard of Shen Zhen residents see problems to conduct a comprehensive prevention and control.ObjectiveTo understand the situation of blood glucose control in people with diabetes before and after the epidemic, analyze its influencing factors, explore the internal relationship between diabetes and risk factors caused by the epidemic, and put forward feasible improvement and control measures.MethodsFrom 2019 to 2021 were retrospectively analyzed in Shen Zhen recreation of diabetes patients of community health service center, into the eligible cases, collect the basic information and related clinical data, comparison of community diabetes patients before and after the outbreak of diabetes control,follow-up visits, and The Times of physical activity, waist circumference, body mass index, blood pressure, uric acid and other clinical data.According to whether blood glucose reached the standard or not during the epidemic period, the patients were divided into the standard group and the unqualified group, and the related factors affecting blood glucose control between the two groups were analyzed.Results1.General information of study subjects: A total of 102 community patients with diabetes were included in this study, including 58 male patients(56.86%).2.Comparison of clinical data of enrolled diabetic patients in 2019 and 2021:(1)Blood glucose control rate: The blood glucose control rate in 2021 was slightly lower than that in 2019(78.43% VS 88.24%);The mean blood glucose control level in 2021 was slightly higher than the mean blood glucose level in 2019(6.73mmol/ L VS 6.67 mmol/ L).(2)Annual patient visits and follow-up rate: the average annual outpatient visits in 2021 were higher than the average outpatient visits in 2019(7.30VS3.29).The annual patient interview rate in 2021 was significantly lower than that in 2019(61.28% vs 84.53%);The annual patient telephone visit rate in 2021 was higher than that in 2019(38.72% vs15.47%).(3)Body Mass Index: the average waist circumference of diabetic patients in2021 was slightly lower than that of diabetic patients in 2019(88.12 cm VS88.65cm).The BMI level of the subjects in 2021 was still slightly lower than that of the subjects in 2019(24.66 kg/m2 vs 25.34 kg/m2).3.Analysis of influencing factors of blood glucose control in community diabetes patients in 2021:(1)There are significant differences in blood glucose control rates according to occupations, and the level of control rates is from high to low:Business/service personnel(92.86%)> professional and technical personnel(85.71%)>unemployed persons(81.82%)> self-employed persons(75%)> retired persons(69.23%)>production and transportation related personnel(57.14%).(2)Average number of telephone visits: the average number of telephone visits in the unqualified group was higher than that in the qualified group(3.45 VS 2.25).(3)Physical activity patterns: There were also differences in the control of blood glucose in different daily activity patterns, and the control level was from high to low: leisure physical activity patterns(87.50%)>traffic physical activity patterns(80.0%)> work physical activity patterns(70.0%)> static sexual life patterns(64.52%).(4)The blood glucose control rates of diabetic patients were94.34%,72.0% and 50%,respectively,when blood glucose was monitored within 1 month,1-6 months and 6-12 months.4.Beneficial factors for blood glucose control of community diabetes patients under the COVID-19 in 2021: patients who engage in leisure sports activities in daily life have better blood glucose control(OR=0.30),and blood glucose monitoring is frequent(OR=0.09).Conclusion1.The blood glucose control rate of community diabetes patients in 2021 decreased compared with that in 2019(78.43% vs 88.24%).2.Leisure physical activity and high frequency blood glucose monitoring are favorable influencing factors for blood glucose control of community diabetes patients under the novel coronavirus pneumonia epidemic. |
