Research On The Current Situation And Countermeasures Of "5+3" Order Oriented Medical Students In Hebei Province

Abstract/Summary:
In order to solve the medical and health problems of rural grassroots residents and further meet the health needs of the people,the country has implemented a series of measures to enhance the capacity of rural medical services and improve the level of rural medical services.At present,more than70000 undergraduate medical students nationwide have received targeted medical student training through orders,and these standardized trained students have played an important role at the grassroots level.This topic uses literature research and survey research methods to study the current cultivation status of some order oriented medical students trained by a certain medical university in Hebei Province in three stages.It explores the professional status,career development,and recognition,attitude,and performance of policy objectives of order oriented medical students at different stages,analyzes the existing problems and reasons,and aims to improve the free cultivation effect of order oriented medical students in Hebei Province,Provide reference and guidance for improving the free training system for order oriented medical students.The first chapter analyzes the order-oriented medical student training policy.Based on the policy documents released on the official website,it will be collected and sorted out from 2010 to 2022 policy documents on the free training of order-oriented medical students,and it can be found that most of the promulgation of order-oriented related policies in China involves multi-department cooperation and cooperation,and the result is that the implementation of the policy is guaranteed,but the specific implementation is difficult,and it is necessary to achieve clear rights and responsibilities,and there is still a certain distance from the formation of a complete policy system.The second chapter conducts statistical analysis on the collected survey questionnaires.During the undergraduate stage,a total of 215 students from three groups of students in grades 18 to 22,including those in grades 18,19,and20,were selected as the survey subjects.The research results showed that the students had the highest satisfaction score for teaching staff(4.31 ± 0.702)and the lowest satisfaction score for their own learning outcomes(3.98 ± 0.816);The first reason why students choose to fill out orders for targeted medical majors is that they have job security after graduation(51.0%);Through multiple linear regression analysis,it was found that the main factors affecting students’ satisfaction with teaching quality include gender,grade,per capita annual income of families,understanding of policies,and expected employment prospects.During the standardized training phase for resident physicians,a total of 49 targeted medical students from all 16 and 17 levels who have participated in residential training for one year were selected as the research subjects.The research results showed that there were fewer students willing to return to grassroots work after residential training,accounting for 37% of the total number,lower than the 55.3% proportion of students on campus;The first reason why students choose to fill in order oriented clinical medicine majors is due to low scores(56.5%);There were statistically significant differences in satisfaction scores among students who had passed the licensed physician examination,were willing to work at the grassroots level,whether residential training was helpful in improving their overall abilities,and had different job prospects(P<0.05).In the grassroots work stage,the research subjects were 12 doctors who studied order oriented clinical medicine at a medical university in Hebei Province and had completed three years of standardized training for resident physicians.The research results showed that there were still some problems in salary,job content,and continuing medical education.The third chapter finds out the problems in the current order-oriented medical student training process.At the national level,there are problems such as inadequate policy publicity,insufficient details of policy content,insufficient capital investment,and lack of good coordination among executive agencies at all levels;At the social level,there are some problems,such as the low attention paid by some contracting units to order-oriented medical students,the lack of medical colleges and universities established in rural areas,the unclear work content of primary medical institutions,and the lack of management;At the personal level,there are some problems,such as lack of continuity,lack of internal learning motivation,lack of professional identity,and weak employment will.The fourth chapter puts forward suggestions on the existing problems on the basis of the third chapter,providing reference for the follow-up improvement of the order-oriented medical students’ free training system.At the national level,we should clarify the policy objectives and unify the policy caliber;Accurately and deeply publicize the policy for key groups and specific targets;Formulate systematic policy plans and improve relevant supporting policies;Emphasize the implementation path and responsibility of policies that are difficult to implement,and strengthen the evaluation of policy effects and power supervision;Use funds reasonably and improve the utilization rate of funds;Promote the links between local governments and social organizations to jointly promote the implementation of policies.At the social and personal level,we should improve the whole society’s awareness of the order-oriented medical student training policy;Building medical colleges and universities in rural areas;Optimize the work content of grass-roots units;Deepen investigation and research,and rely on social forces to solve practical problems;Clarify learning motivation and enhance professional identity.
