Clinical Observation Of Sanhuang Yarn Strip In The Postoperative Treatment Of High Complicated Anal Fistula

Abstract/Summary:
Objective: This study observed the clinical therapeutic effect of Sanhuang Yarn Strip on high complicated anal fistula,explored its effectiveness in improving postoperative pain and edema,reducing exudation and accelerating wound healing,and evaluated the safety of its clinical application,so as to provide scientific basis for clinical application of Sanhuang Yarn Strip to treat common complications after high complicated anal fistula.Methods: Sixty-two patients who were diagnosed with high complex anal fistula and admitted to the Department of Anorectal Medicine of the Affiliated Hospital of Gansu University of Traditional Chinese Medicine between January 2022 and December 2022 and met the inclusion criteria were selected and divided equally into the control group and the treatment group according to the randomization principle,with 31 patients in each group,both groups were treated with high complex anal fistulae by incision and hanging under lumbar anesthesia,and the dressing was changed on the second day after surgery.In the control group,Vaseline gauze was applied to the wound after fumigation and disinfection,and in the treatment group,Sanhuang Yarn Strip was applied to the wound once a day until the patients were discharged from the hospital.The pain,edema and granulation growth scores of the two groups were recorded separately,the amount of exudate was calculated,the healing time of the wounds was counted,the treatment effect was analyzed,and the safety of the clinical application of Sanhuang Yarn Strip was also evaluated,and the data obtained were finally compiled and statistically compared.Results:1.There was no difference in gender,age and number of fistulas between the two groups(P > 0.05),allowing for comparison.2.Degree of trauma pain: Comparison of pain between the two groups before replacing the dressings,P > 0.05,was not statistically significant,indicating that there was no difference in trauma pain between the two groups before replacing the dressings,which could be compared;comparison of pain scores between the two groups on postoperative days 3,7and 14,P < 0.05,was statistically significant.Pain was relieved in both groups on postoperative day 3 compared to before the change of dressings,day 7 compared to day 3,and day 14 compared to day 7,with P values < 0.05,and the pain relief was greater in the treatment group,indicating that the effect of Sanhuang Yarn Strip in relieving postoperative pain was significantly better than that of petroleum jelly gauze.3.Granulation growth score: Comparison of granulation growth between the two groups before replacing dressings and on postoperative day 3,P > 0.05,no statistical significance,indicating that there is no difference in granulation growth between the two groups before replacing dressings and on postoperative day 3,which can be compared;on postoperative day7 and 14,comparison of granulation growth score between the two groups,P < 0.05,the difference is statistically significant.The granulation tissue improved in both groups on postoperative day 7 compared with day 3 and day 14 compared with day 7,with P values<0.05,and the improvement of granulation tissue growth was more obvious in the treatment group,indicating that the role of Sanhuang Yarn Strip in promoting granulation growth was significantly greater than that of petroleum jelly gauze.4.Edema scores: Comparison of edema scores before replacing auxiliary materials in the two groups,P > 0.05,no statistical significance,indicating that there was no difference in edema at the trabecular margin between the two groups before replacing auxiliary materials,which could be compared;comparison of edema scores in the two groups on postoperative days 3,7 and 14,P < 0.05,the difference was statistically significant.In both groups,edema was relieved on postoperative day 3 compared to before the change of dressings,day 7compared to day 3,and day 14 compared to day 7,with P values < 0.05,and the degree of edema relief was greater in the treatment group,indicating that the effect of Sanhuang Yarn Strip in relieving postoperative edema was significantly better than that of petroleum jelly gauze.5.Trabecular exudate volume: The comparison of trabecular exudate volume between the two groups before replacing dressings and on day 3,P > 0.05,was not statistically significant,indicating that there was no difference in trabecular exudate between the two groups before replacing dressings and on postoperative day 3,which could be compared;on postoperative days 7 and 14,the comparison of trabecular exudate volume between the two groups,P < 0.05,was statistically significant.The amount of traumatic exudate decreased in both groups on postoperative day 7 compared with day 3 and day 14 compared with day 7,and the P value was <0.05,and the effect of reducing exudation was more obvious in the treatment group,indicating that the efficacy of Sanhuang Yarn Strip in reducing exudation was significantly higher than that of petroleum jelly gauze.6.Wound healing time: The difference was statistically significant when comparing the healing time of wounds in the two groups after treatment,P < 0.05.The average healing time was 34 days in the treatment group and 43 days in the control group,indicating that the effect of Sanhuang Yarn Strip in promoting wound recovery and shortening healing time was significantly better than that of Vaseline gauze.7.Efficacy evaluation: On the 14 th postoperative day,the effective rate of Sanhuang Yarn Strip was 100% and that of Vaseline gauze was 83.33%,which was analyzed at P < 0.05 and was statistically significant,indicating that the clinical treatment effect of Sanhuang Yarn Strip was more significant than that of Vaseline gauze for improving the common postoperative complications.8.Clinical safety aspects: No adverse reactions occurred with both Sanhuang Yarn Strip and Vaseline gauze.Conclusions:1.Sanhuang Yarn Strip is used for postoperative high complex anal fistula to effectively relieve postoperative pain and edema and reduce traumatic exudation.2.The use of Sanhuang Yarn Strip for high complex anal fistula significantly promoted granulation growth,accelerated healing and shortened wound recovery time.3.The results of this study can confirm that the clinical efficacy of Sanhuang Yarn Strip for postoperative high-grade complex anal fistula is significant,and there are no adverse reactions,and the safety is high.
