Objective:To observe the effect of wrist-ankle acupuncture on pain, anxiety and sleep in patients with postoperative pain of patella fracture, and to provide appropriate technology of traditional Chinese medicine nursing for auxiliary treatment of postoperative pain of patella fracture.Methods:84 patients with pain after patellar fracture surgery who met the inclusion criteria were randomly divided into wrist-ankle acupuncture group and control group, each group for 42 cases. On the basis of the control group,wrist-ankle acupuncture therapy was added. The changes of visual analogue pain score(VAS),safety observation indicators(broken needle, needle body deformation)were observed in patients with postoperative pain of patellar fracture, efficacy evaluation, salvage analgesia, anxiety self-rating scale(SAS),and Athens sleep scale(AIS),and the adverse reactions during treatment of the two groups were recorded. All data were processed using SPSS26.0 statistical software to provide an objective evaluation of the study.Results:(1)Comparison of VAS scores of patients in the two groups:before the intervention, there was no significant difference in VAS scores(P>0.05)between the wrist-ankle acupuncture group and the control group, and after the intervention, the scores of the two groups were statistically significant(P<0.05);within the group, the control group’s VAS score was not statistically significant(P>0.05)before treatment(t0)to 12h(t1),48h(t3)after surgery to 72h(t4)after surgery(P>0.05),and other differences were statistically significant(P<0.05).(2)Use of salvage analgesics:the number of cases of salvage analgesics used in the wrist-ankle acupuncture group was lower than that in the control group, and the difference was statistically significant(P<0.05).(3)Comparison of SAS scores of patients in the two groups:before the intervention, there was no significant difference in SAS scores(P>0.05)between the wrist-ankle acupuncture group and the control group, and after the intervention, the scores of the two groups were statistically significant(P<0.05);within the group, the control group’s SAS score was not statistically significant(P>0.05)before treatment(t0)to 12h(t1),48h(t3)after surgery to 72h(t4)after surgery(P>0.05),and other differences were statistically significant(P<0.05).(4)Comparison of AIS scores of patients in the two groups:before the intervention, there was no significant difference in AIS scores(P>0.05)between the wrist-ankle acupuncture group and the control group, and after the intervention, the scores of the two groups were statistically significant(P<0.05);within the group, the control group’s AIS score was not statistically significant(P>0.05)before treatment(t0)to 12h(t1),48h(t3)after surgery to 72h(t4)after surgery(P>0.05),and other differences were statistically significant(P<0.05).(5)Analysis of clinical efficacy of patients in the two groups:after different interventions, pain was improved in both groups(the total effective rate of the wrist and ankle needle group was 95.56%,the total effective rate of the control group was 80.95%,and the wrist-ankle acupuncture group was higher than that of the control group),and the analysis of rank-sum test showed that the pain relief in the wrist-ankle acupuncture group was better than that in the control group(P<0.05).(6)Safety evaluation of patients in the two groups:in the process of intervention,2 cases of needle sickness occurred in the wrist and ankle needle group, and only 1 case in the control group had abdominal distension, which relieved itself after rest, belonging to safety evaluation level II,and the rest of the patients belonged to safety evaluation level I,and the safety comparison between the two groups was not statistically significant(P>0.05).Conclusion:Compared with conventional care,wrist-ankle acupuncture can effectively reduce pain symptoms, reduce the number of remedial analgesia, improve anxiety and sleep in patients with patellar fracture surgery, and improve the cure rate of pain patients.
