Abstract/Summary:
Objective:1.Based on data mining methods,analyze Professor Zhang Shiqing’s medication patterns and prescription compatibility characteristics in the treatment of constipation in children with intestinal dryness and fluid deficiency,and summarize his academic ideas and clinical experience;2.Through clinical research,observe and evaluate the clinical efficacy of Maren Shengdi Tang in treating infantile constipation caused by intestinal dryness and deficiency of fluid,in order to provide reference for the treatment of infantile constipation with traditional Chinese medicine.Methods:1.Collect medical records of pediatric constipation caused by intestinal dryness and Tianjin deficiency treated by Professor Zhang Shiqing at the Ming Traditional Chinese Medicine Workstation of the Affiliated Hospital of Gansu University of Traditional Chinese Medicine from November 2020 to January 2022,select effective prescriptions,and input them into Microsoft Office Excel 2019 to establish a prescription database.Use the Traditional Chinese Medicine Inheritance Calculation Platform(V3.0)to statistically analyze the gender,efficacy,frequency,and meridian distribution of the children,as well as the four qi and five flavors,Then,SPSS modeler 18.0 and IBM SPSS Statistics 24.0 were used to perform association rules and cluster analysis on prescriptions.Based on the results of prescription data mining,the medication patterns of Professor Zhang Shiqing’s prescription for treating constipation in children with intestinal dryness and Tianjin deficiency were analyzed,and their academic ideas and clinical experience were summarized.2.Based on the diagnosis and inclusion criteria of this study,76 children with intestinal dryness and Tianjin deficiency constipation were selected.The treatment group consisted of38 cases who received oral administration of Maren Shengdi Tang,while the control group consisted of 38 cases who received oral administration of Bifidobacterium tetravalent live bacterial tablets.Both groups received 2 courses of treatment(6 days per course,with 1 day off between the 2 courses),and were followed up 4 weeks after the end of treatment.Timely record the individual symptom scores of the patient before,after treatment,and during follow-up,calculate the main and secondary symptom scores and total scores.After follow-up,conduct statistical analysis of the results,and compare and evaluate the clinical efficacy and differences between the two groups of treatment.Results:1.Analysis of the medication patterns of Professor Zhang Shiqing in the treatment of constipation in children with intestinal dryness and fluid deficiency(1)This study ultimately included 104 children,with male and female children accounting for 52.85% and 47.15% respectively.A total of 123 effective traditional Chinese medicine prescriptions were selected;(2)The 123 prescriptions involved a total of 125 traditional Chinese medicines,with a total frequency of 2367 uses of traditional Chinese medicines in the formula.Among them,there were 26 herbs with a total frequency of ≥ 30,and the top 5 were Huomaren,Angelica sinensis,Raphani seed,Fructus Aurantii,and Fructus Trichosanthes;It mainly involves 18 types of efficacy,with higher frequency including tonifying deficiency,clearing heat,dissipating food,resolving phlegm,relieving cough and asthma,regulating qi,and reducing diarrhea.The top three are tonifying deficiency,clearing heat,and dissipating food;(3)The properties of 125 drugs are mainly cold,calm,and warm,with sweet and bitter flavors,mainly targeting the spleen,stomach,lungs,and large intestine meridians;(4)By analyzing the association rules of the group,a total of 34 drug association rules were obtained,including 16 second-order drug association rules and 18 third-order drug association rules.Commonly used drug combinations include: Huomaren Danggui,Huomaren Raphani Zi,Huomaren Fructus Aurantii,etc;(5)Cluster analysis was conducted on 26 high-frequency drugs,and when they were divided into 6 cluster groups,the specific group of traditional Chinese medicine was: C1 group,Huomaren;C2 group,Angelica sinensis,roasted licorice;C3 group,radish seed,sour jujube seed,cypress seed,malt,hawthorn,Shenqu,Polygala tenuifolia,chicken inner gold;C4 group,Fructus Aurantii,Cimicifuga,Radix Rehmanniae,Radix Paeoniae Rubra,Radix Paeoniae Alba,Radix Scutellariae Scutellariae;C5 group,Gualouzi,Houpu,Peach Kernel,Bitter Almond,Scrophulariae,Rhubarb;C6 group,tangerine peel,atractylodes macrocephala,betel nut.2.Clinical observation on the therapeutic effect of Ma Ren Sheng Di Tang on constipation in children with intestinal dryness and fluid deficiency type(1)Main and secondary symptom scores and total scores: After two courses of treatment,the main and secondary symptom scores and total scores of the two groups of children were significantly reduced compared to before treatment(P<0.001);Comparison between groups found that the treatment group was superior to the control group in improving the main and secondary symptom scores and total scores of the children(P<0.05);(2)Symptom scores for each individual symptom: After treatment,it was found that the symptom scores for each individual symptom in the treatment group were significantly reduced(P<0.001),while the symptom scores for fecal characteristics,difficulty in defecation,duration of defecation,frequency of defecation,bloating,abdominal pain,and decreased appetite in the control group were significantly lower than before(P<0.001).However,there was no significant difference in symptom scores for dry mouth,excessive drinking,and short and red urine(P>0.05);There was no significant difference between the two groups in improving defecation duration,abdominal pain,and decreased appetite(P>0.05),but there was a statistically significant difference in improving defecation frequency,difficulty in defecation,fecal characteristics,and abdominal distension(P<0.05).There was a significant difference in improving dry mouth,excessive drinking,and short and red urine(P<0.001);(3)Overall efficacy and recurrence rate: After treatment,the total efficacy rate of the treatment group reached 91.89%,which was higher than 78.38% in the control group,indicating that the overall efficacy of the treatment group was better than that of the control group;After 4 weeks of follow-up after treatment,the recurrence rate in the treatment group was 11.76%,lower than 41.38% in the control group.Conclusion:1.Professor Zhang Shiqing’s treatment of constipation in children with intestinal dryness and deficiency of fluid is based on a combination of clear and nourishing drugs,as well as nourishing and nourishing the spleen and stomach.Clinical treatment often follows the principles of nourishing yin and blood,moistening the intestines and promoting bowel movements.According to the different symptoms of the children,treatment methods such as clearing heat and purging fire,dissipating food and resolving accumulation,calming the liver and stomach,and regulating the qi mechanism are supplemented;2.Both oral Maren Shengdi Tang and Bifidobacterium quadruplex live bacterial tablets can treat constipation in children with intestinal dryness and fluid deficiency.However,Maren Shengdi Tang is more effective than Bifidobacterium quadruplex live bacterial tablets in improving single symptoms such as fecal characteristics,difficulty in defecation,frequency of defecation,bloating,dry mouth,excessive drinking,and short and red urine,and has a higher overall effective rate and lower recurrence rate.
