A Study On The Relationship Between Latent Profile Analysis Of Benefit Finding In Colorectal Cancer Patients With Enterostomy And Stomy Adaptation

ObjectiveThe purpose of this study was to explore the potential categories of benefit finding in colorectal cancer enterostomy patients,in order to identify heterogeneous subgroups of benefit finding in this population,and on this basis to explore the influencing factors of benefit finding potential categories in colorectal cancer enterostomy patients,and compare the differences between different potential categories in terms of stomy adaptation in colorectal cancer enterostomy patients.MethodsA total of 216 patients with colorectal cancer enterostomy who met the inclusion and exclusion criteria of a Grade-A hospital in Qingdao from January 2022 to September 2022 were chosen as the objects of study,and a convenience sampling method was adopted.Social demographic characteristics(age,gender,education level,marital status,home address,residence status,income level,etc.)and ostomy-related characteristics(operation time,type of ostomy,whether there are ostomy complications,etc.)of the subjects were collected by self-designed general data questionnaire.Benefit Finding Scale(BFS),Medical Coping Modes Questionnaire(Medical Coping Modes Questionnaire,MCMQ),Perceived Social Support Scale(PSSS),Life Orientation Test-Revised,LOT-R),Event Related Rumination Iventory(ERRI),Ostomy adjustment inventory-20,OAI-20)was used as a survey tool to assess benefit findings,coping styles,social support,optimistic tendencies,rumination,and ostomy adaptation in colorectal cancer patients.SPSS 26.0 and Mplus 8.3 were used for statistical analysis of the data.Descriptive analysis,potential profile analysis(LPA),Chi-square test,one-way analysis of variance,ordered multiple Logistic regression and multiple linear regression were used for statistical analysis.Results1.The total benefit of colorectal cancer enterostomy was(73.03±18.38),which was above the medium level.The average score of each dimension in descending order was:personal growth(22.91±6.90),world outlook(12.12±3.31),health behavior(11.04±3.27),social relationship(10.32±2.92),acceptance(9.41±2.96)and family relationship(7.22±2.19).2.There was significant population heterogeneity in colorectal cancer enterostomy benefit findings,which were divided into three potential categories: "high benefit findingshealthy and loved group"(67 cases(31%),"moderate benefit findings-physical inactivity group"(113 cases(52.3%),and "low benefit findings-coping fatigue group"(36 cases(16.7%).3.Univariate analysis showed that,There were statistically significant differences among the potential categories of benefit found in different colorectal cancer enterostomy patients(all P < 0.05),including occupation,family per capita monthly income,ostomy years,ostomy type,communication degree with medical personnel,coping style total score and scores of each dimension,social support total score and scores of each dimension,optimistic tendency score and invasive rumination score.Orderly multiple Logistic regression analysis showed that occupation,degree of communication with medical staff,coping style,optimistic tendency,and social support were characteristics of potential categories of benefit found in different colorectal cancer enterostomy patients.Retirement(OR=3.053,P=0.045),optimistic tendency(OR=1.126,P=0.015),family support(OR=1.122,P=0.021),other support(OR=1.124,P=0.024),coping style(OR=1.203,P<0.001)patients with higher scores were more likely to be in the "high benefit findings-healthy and loved group";Patients who never(OR=0.092,P=0.014)and rarely(OR=0.144,P=0.012)communicated with health care providers and had higher yield coping styles(OR=0.754,P=0.001)were more likely to be in the "low benefit finding-coping fatigue group".4.Multiple linear regression results showed that the stoma adaptation scores of colorectal cancer enterostomy patients in the "high benefit findings-healthy and loved group" and "moderate benefit findings-physical inactivity group" were significantly higher than those in the "low benefit findings-coping fatigue group"(all P<0.05).However,there was no significant difference in the scores of stoma adaptation between the "high benefit finding-healthy loved group" and the "moderate benefit findings-physical inactivity group"(P>0.05).Conclusion1.The benefit of colorectal cancer enterostomy was found to be moderate to upper level.2.There was significant group heterogeneity in the benefit findings of colorectal cancer enterostomy patients,which could be divided into three categories: "high benefit findingshealthy and loved group","moderate benefit findings-physical inactivity group" and "low benefit findings-coping fatigue group".3.Occupation,degree of communication with medical staff,coping style,optimistic tendency,and social support are different among different potential categories of benefit finding in colorectal cancer enterostomy patients,which can predict the classification of potential categories of benefit finding.4.Compared with the "low benefit findings-coping fatigue group",the "high benefit findings-healthy and loved group" and "moderate benefit findings-physical inactivity group" patients had a higher level of ostomy adaptation,but there was no significant difference between the "high benefit findings-healthy and loved group" and "moderate benefit findings-physical inactivity group" patients.
