| ObjectiveThe cross-sectional survey method was used to understand the status quo of stoma adaptation of patients with enterostomy,and to analyze the influencing factors of the adaptation level of patients with enterostomy;at the same time,to understand the status quo of family care,mindfulness level and disease benefit sense of patients with enterostomy and the correlation between them and stoma adaptation;in order to provide reference for improving the stoma adaptation level of patients with enterostomy.MethodsFrom August 2018 to February 2019,the convenience sampling method was used to select the patients with enterostomy who met the acceptance and discharge standard in a third class a hospital in Dalian as the research object.The convenience sampling method was used to select 180 patients with enterostomy who met the acceptance and discharge standard in a third class a hospital in Dalian.The general information questionnaire,the social psychological adaptation scale of patients with enterostomy and the family care index questionnaire were used,mindfulness and attention awareness scale,disease benefit scale were investigated and analyzed.Excel was used to build database,spss24.0 was used for statistical analysis,and AMOS24.0 software was used to build structural equation model.Descriptive or single factor analysis of variance,t-test,Pearson correlation analysis,multiple linear regression,intermediary effect analysis and path analysis were used to analyze the collected data.Results1.The score of colostomy adaptation was(45.82 ± 8.81),which was in the middle and low adaptation level,48 cases(26.7%)in the low adaptation level,118 cases(65.6%)in the middle adaptation level and 14 cases(7.8%)in the high adaptation level.2.Family care score(6.96 ± 1.47);total mindfulness score(61.83 ± 7.63).The highest score was personal growth(17.66 ± 3.22),healthy behavior(12.81 ± 1.53),and the lowest score was worldview(7.44 ± 1.69).3.The results of person correlation analysis showed that: the total score of stoma adaptation was positively correlated with the total score of family care(r = 0.526 P <0.001);it was positively correlated with the total score of mindfulness(r = 0.726 P <0.001);it was positively correlated with the total score of disease benefit(r = 0.564 P <0.001).4.Univariate analysis results:there were significant differences among different genders,education level,marriage status,family location,living status,occupation,family per capita monthly income(yuan),time after operation(year),communication level,coping style,complications,and frequency of attending social gatherings(P < 0.05).5.The results of multiple linear regression analysis:the multiple linear regression analysis of the influencing factors of stoma adaptation of patients with enterostomy.There are four factors in the regression equation:the level of family per capita monthly income,the length of time after enterostomy,the level of mindfulness,and the sense of disease benefit,which jointly explain 57.5% of the variation.6.The results of mediating effect test:the mediating effect of mindfulness between stoma adaptation and disease benefit in patients with enterostomy is 59.89% of the total effect.7.The final fitness indexes of the structural equation model constructed by AMOS are CMIF / DF = 2.46,GFI = 0.901,CFI = 0.924,AGFI = 0.917,IFI = 0.924,RMSEA =0.075.All the fit indexes are within the acceptable range,indicating that the model fit degree is good and the model is acceptable.Bootstrap test showed that the 95%confidence interval of the total effect,direct effect and indirect effect of disease benefitsense on stoma adaptation was not 0,and Z value was greater than 1.96,indicating that mindfulness played a mediating role between disease benefit sense and stoma adaptation,and it was a partial mediating role.Conclusions1.The stoma adaptation of patients with enterostomy is at a low and medium level,which needs to be improved.2.The consciousness and disease benefit of patients with enterostomy can also affect the level of stoma adaptation.The higher the score of consciousness and disease benefit,the higher the level of stoma adaptation,and the family care has a positive predictive effect on it.3.The stoma adaptation level of patients with enterostomy is affected by different gender,education level,marital status,family location,living status,occupation and other factors.We should focus on the patients with low income and less than 1 year or more than 3 years after operation,and take effective nursing measures according to their characteristics.4.Mindfulness plays a mediating role between stoma adaptation and disease benefit in patients with enterostomy,and can indirectly affect the level of stoma adaptation in patients with enterostomy. |
