Abstract/Summary:
Objective: Through on-site investigation, this study understands the current situation of shame, mental resilience and quality of life. And the population difference analysis, using the collected survey data to further explore the impact of shame and mental elasticity on the quality of life,On the basis of this,we propose measures to reduce the patient’s sense of shame, enhance personal mental resilience and improve the quality of life,To provide guidance for the clinical diagnosis and treatment of dermatology in hospitals,Finally, the purpose of enhancing the sense of life of psoriasis patients was achieved. Methods: Three hospitals in Shandong province were sampled in January ~ December 2021 using convenient sampling method.Qingdao University Affiliated Hospital,Weifang People’s Hospital,Binzhou Medical College Affiliated Hospital)167 psoriasis patients conducted questionnaires, disease severity assessments and in-depth personal interviews.Statistical methods such as descriptive analysis,t-test and variance analysis were used to analyze the status quo and population differences of disease stigma, psychological resilience, quality of life. The effect of shame and mental elasticity on quality of life was further investigated by using multiple stratified regression.Results:(1)83.83% of patients with normal psoriasis had an average course of 7.28 ± 8.32 years.83.83% of patients reported itching symptoms, with 41.92% of mild itching and 21.56% of moderate itching. In addition,68.26% reported having experienced discrimination due to skin disease. The surface area of involvement varies from 1% to 93%.On average,36.34±25.12%,2.48% of patients were affected "< 3%" and 16.15% "3%≤BSA<10%," 81.37% "≥10%." According to PASI values,The most severe patients were 36.53%.The least moderate was 31.14%.(2)The stigma score was 2.79±0.68,The differences in the itching of skin, the degree of itching of skin, the discrimination and the degree of discrimination were statistically significant(P<0.05);The mental elasticity score was3.65±0.79,and differences in marital status were statistically significant(P<0.05);Quality of life score of 72.02±17.58,different skin itching degree, chronic disease, discrimination, discrimination degree of statistical significance(P<0.05).(3)The quality of life of psoriasis sufferers is negatively related to the feeling of shame, and the four dimensions of shame(Premonition and rejection, sensitivity and defects, positivity, hiding)Negative correlation, respectively, after excluding the influence of the control variable,Stigma negatively affects the quality of life, and the interpretation of the quality of life is 16.5%;the quality of life of psoriasis patients is positively related to mental elasticity.Three dimensions of mental resilience(self-strength,optimism,toughness)There was positive correlation, and after excluding the influence of control variables, mental elasticity positively affected the quality of life, and the interpretation of quality of life was 37.3%.Conclusion: High levels of skin itching, chronic disease, discrimination and high levels of discrimination are risk factors for decreased quality of life in psoriasis patients.Shame and mental elasticity are the key factors influencing the quality of life. The higher the shame, the worse the quality of life, the higher the mental elasticity, the better the quality of life. The survey showed that psoriasis patients’ survival status is not optimistic, only focus on the physiological and psychological effects of the optimal treatment of the disease, further reduce the patient’s sense of shame, enhance the patient’s individual psychological resilience, can achieve long-term relief, improve the quality of life effect.
