Clinical Observation On The Therapeutic Effect Of Modified Huanglian Wendan Decoction On Damp-heat Syndrome Of Chronic Kidney Disease In Stage 3

Objective:To explore the clinical efficacy and mechanism of modified Huanglian Wendan Decoction in the treating of patients with damp-heat syndrome in phase 3 of chronic kidney disease,to carry out clinical research and to evaluate its clinical effect through quantitative indicators,to open up new ideas for the treatment of chronic kidney disease in TCM,and to provide scientific basis for it.Method:Select 60 patients with chronic kidney disease in phase 3 that meet the criteria for inclusion and the diagnosis of damp-heat syndrome according to TCM syndrome differentiation.According to the principle of randomization,parallelism and control,they are divided into treatment group and control group by random number table,with 30 patients in each group.The control group was given the conventional treatment plan of combined application of Corbrin Capsule,and the experimental group was added with a Chinese medicine decoction called modified Huanglian Wendan Decoction on the basis of the control group.8 weeks as a course of treatment.Observe and record all laboratory indicators,TCM syndrome scores and safety indicators before and after treatment in both groups.Result:1.Single score of TCM syndrome: comparing the single score of TCM syndrome of patients in the test group and the treatment group before and after treatment,it was found that both groups could improve the symptoms(P<0.01).By comparing the scores of single TCM syndromes of the two groups after treatment,it was found that there was no statistically significant difference between the two groups in improving the symptoms of dry mouth(P>0.05).The test group was significantly better than the control group in improving the symptoms of nausea and vomiting,body weight and limb sleepiness,lack of appetite and stupidity,bitter mouth,abdominal distension,and greasy mouth(P<0.01).2.Total score of TCM syndrome: compared with the group before and after treatment,the total score of TCM syndrome in the two groups had significant statistical significance(P<0.01);After treatment,the total score of TCM syndromes in the test group was significantly lower than that in the control group,with a statistically significant difference(P<0.01).3.Effect of TCM syndrome: comparison of efficacy of TCM syndrome between groups after treatment,the total effective rate of the control group was 63.3%,and the total effective rate of the test group was 86.7%,the difference was statistically significant(P<0.05).4.Laboratory indicators: compared with the intra-group before and after treatment,all laboratory indicators of the patients in the test group were improved(P<0.05),and the serum creatinine,blood urea nitrogen,uric acid,estimated glomerular filtration rate of the patients in the control group were significantly different from those before treatment(P<0.05).After treatment,the effect of the test group in reducing UA,Scr and improving e GFR was better than that of the control group(P<0.05).There was no significant difference between the two groups in improving hemoglobin and albumin(P>0.05).It shows that the modified Huanglian Wendan Decoction has a good effect on improving the renal function of chronic kidney disease.5.Total clinical efficacy: the total effective rate of the test group is93.3%,and that of the control group is 70.0%.The statistical analysis results show that the modified Huanglian Wendan Decoction has more advantages in clinical effective rate.(P<0.01)6.Safety: During the treatment,the general vital signs of the patients in the test group and the control group were always in a stable state.It was found that the routine electrocardiogram of the patients in the two groups before and after the treatment were basically normal,and there were no significant abnormal changes in the three routine and liver function tests,and there were no adverse events during the study.Conclusion:1.Modified Huanglian Wendan Decoction can effectively reduce the serum creatinine and uric acid of patients,protect the residual nephron,delay the progress of the disease,and improve the survival rate of patients with chronic kidney disease.2.Modified Huanglian Wendan Decoction has the effect of increasing hemoglobin and albumin to a certain extent,and is helpful to improve anemia and hypoproteinemia in patients with chronic kidney disease.3.The prescription of modified Huanglian Wendan Decoction is based on the viscera and focuses on the triple energizer.It can effectively improve the clinical symptoms of TCM in patients with chronic kidney disease with damp-heat syndrome in phase 3.It has no obvious adverse reactions and good safety after long-term use.
