Purpose:By collecting clinical cases of migraine that meet the natrol standard,statistical the disease and syndrome information of patients before and after intervention,using the single factor Chi-square test and binary Logistic regression analysis,to explore the core treatment points of Angelica Sini decoction in the treatment of blood deficiency and cold coagulation type migraine.In order to provide scientific identification basis for the majority of TCM colleagues to use this prescription in the treatment of blood deficiency cold coagulation type migraine syndrome differentiation.Method:The medical records of patients with blood deficiency and cold coagulation type migraine were collected and sorted out according to the standard and unified format in tutor Huang Chunhua’s clinic.According to the diagnostic criteria of traditional Chinese and Western medicine and the inclusion and exclusion criteria of blood deficiency cold coagulation type migraine,60 cases of blood deficiency cold coagulation type migraine were selected.The patients’basic medical history(course of disease,previous history,etc.),social demographic information(gender,age,occupation,etc.),four TCM diagnosis information(main syndrome,concurrent syndrome,tongue and pulse signs,etc.),intervention plan of Angelica Sini Decoction,migraine attack intensity,attack frequency,duration,accompanying symptoms,other symptoms and improvement of comprehensive score before and after treatment were analyzed.Univariate analysis was used to compare the differences between the effective group and the ineffective group in the case data(basic medical history information,TCM four-clinic information,social demographic information,migraine attack situation scoring scale and TCM migraine syndrome grading scale,etc.)at the first visit.The inclusion criteria was P<0.05,and variable X1 with statistical difference was screened out.The variable X1 selected by the above steps was further analyzed by binary Logistic regression,and the stepwise method was used(including criteria P<0.05,excluding criteria P>0.1).The independent variable was X1,and the dependent variable was effective and invalid.Finally,the variable X2 with statistical difference was obtained(P<0.05),through which the core identification basis of this prescription for the treatment of blood deficiency and cold coagulation type migraine was discussed.Results:1.According to the three time points of follow-up before treatment,after treatment and 1 month after withdrawal of the migraine attack score scale,after 1 month of treatment with Angelica Sini decoction,the five score data of the number of attacks,duration,intensity of attacks,accompanying symptoms and other symptoms of the migraine patients in the diagnosis and treatment group were significantly improved,with P values less than 0.01,and the differences were very significant.The total effective rate was 78.3%after 1 month of treatment,and 75.0%after 1month of follow-up.2.The single factor exploratory analysis finally screened out 9variables with statistical differences.Among them,sinking pulse,cold limbs,white face,undertongue silting,aggravation in cold,white moss,tasteless mouth were positively correlated with the curative effect of Angelica Sini Decoction after intervention,while eating less and ill wind were negatively correlated with the curative effect.Further Logistic regression analysis was carried out on the above statistically significant variables,and the final variables included in the equation were pulse sinking,cold limbs and white face.Conclusion:1.Angelica Sini Decoction has a significant effect on the treatment of blood deficiency and cold type migraine,which can significantly improve the VAS score,headache intensity,frequency of attacks,accompanying symptoms,duration and other symptoms of migraine patients,and effectively improve the manifestations of TCM symptom score of migraine,with fewer adverse reactions and good safety.Under the premise of correct syndrome differentiation,appropriately increasing the course of intervention,improving the follow-up system,and grasping the core identification basis of Angelica Sini Decoction in the treatment of blood-deficiency and cold-coagulation migraine may be beneficial to maintain or improve the curative effect of Angelica Sini Decoction.2.Through the single factor exploration analysis,it is concluded that the effect of Angelica Sini Decoction after intervention is positively correlated with the thin pulse,cold limbs,white face,under-tongue stasis,aggravated cold,white moss and tasteless mouth,while the effect is negatively correlated with the lack of food and adverse wind,suggesting that if the identification basis of the patient’s initial diagnosis information includes the thin pulse,cold limbs,white face,under-tongue stasis,aggravated cold,white moss and tasteless mouth,In addition,the therapeutic effect of this diagnosis and treatment plan may be more significant when the two syndrome information of hypoxia and ill wind are excluded.Further Logistic regression analysis showed that the core identification criteria of Angelica Sini decoction application might be thin pulse,cold limbs and white face.3.Through retrospective exploratory analysis,this study explores the hidden information and potential rules in the differentiation and treatment process of clinical excellent doctors,and discusses the best differentiation and treatment points of Angelica Sini Decoction in the treatment of blood-deficiency and cold-coagulation migraine.This research idea and method can be used as reference for future scientific exploration of identification.To some extent,it is conducive to the innovation and standardized development of Chinese medicine.
