Observation On Clinical Efficacy Of Acupuncture Foot Yin And Yang Meridian "Shu" Points In The Treatment Of Knee Osteoarthritis

Objective: In order to evaluate the clinical efficacy of knee osteoarthritis(KOA),this project takes KOA patients with wind chill and dampness symptom as the research object,throughing compares and observes the difference of clinical efficacy of foot yin meridian Shu points and foot yang meridian Shu points in the intervention of wind-cold-dampness arthralgia type KOA patients,find out which one is better for the treatment of knee osteoarthritis,and then pick out more advantageous acupuncture points combination for the acpuncture treatment of KOA with wind-cold-dampness arthralgia type,so as to find out more effective acupuncture prescriptions for the treatment of KOA with wind-cold-dampness arthralgia type,and improve clinical efficacy.Methods: Based on the research purpose and the standards of the project,and this project assignment 60 KOA patients with wind chill and dampness symptom into two groups,respectively are 30 cases in the foot yin meridian Shu points group and 30 cases in the foot yang meridian Shu points group.According to the textbook of Acupuncture Therapy in the“13th Five-Year Plan”,and basis of routine acupoint selection for acupuncture treatment of KOA,this research plus the infusion point,the acupoints in the foot yin meridian Shu points group include: Liangqiu point,Xuehai point,Dubi point,inner knee eye point,Yanglingquan point,Ashi point,Taixi point,Taibai point and Taichong point;the acupoints in the foot yang meridian Shu points group include: Liangqiu point,Xuehai point,Dubi point,inner knee eye point,Yanglingquan point,Ashi point,Xiangu point,Footlinqi point and Shugu point.We need to select acupionts on the grounds of different syndromes of patients.The specific acupuncture operation method is as follows: both groups of patients should took the supine position,and fully exposed the skin of the acupoint area,then disinfected the skin at the acupoint with 75%alcohol cotton balls,if the patients are allergic to alcohol,we should chose iodophor cotton balls for disinfection,for the choice of needles,0.30 mm × 40 mm disposable millineedles were usually used for acupuncture.During the treatment of foot yang meridian Shu points group,the distal Shu point such as Xiangu point、Footlinqi point and Shugu point was first acupunctured with the twisting laxative method,and then the local points was pierced,which include Liangqiu point、Xuehai point、Dubi point、inner knee eye point、 Yanglingquan point and Ashi point with the flat supplement and laxative method.After the needle is pricked,feeling sore and distended,acid and numbness under the needle(this is the so-called feeling of getting qi by acupuncture),then leave the needle in the points for 30 minutes,and rotate the needle body to make up for the flat supplement and laxative method every 15 minutes during the treatment,and after 30 minutes the needle is dispensed.During the treatment of foot yin meridian Shu points group,the distal Shu point such as Taixi point、Taibai point and Taichong point was first acupunctured with the twisting laxative method,the acupuncture methods of the rest of the local points are the same as above.The above two groups patients were treated once a day,all of them were treated 10 times in our project and10 times was a total of 1 course.We choose three scales as the observation index,which are visual analogue scale(VAS)score、realistic pain scale(PPI)score and knee osteoarthritis severity index scale(ISOA)score.When the patient were treated,we should let them fill the three scales.And after them finish all the treatment,we also let them fill the aboce three scales.Then we should gather all the datd and then we used SPSS 25.0 statistical software to statistically analysis the treatment effect.Finally,for the sake of objectively facilitate analyze the discrepancy in clinical efficacy of acupuncture between the above groups before and after the treatment.And then we can choose better acupoints compatibility for the acupuncture treatment of KOA with wind-cold-dampness arthralgia type,so that it can afford scientific evidence to better treat KOA.Results:1.After statistical analysis of the date,there was no obvious difference in age distribution,gender composition,the length of the disease,VAS score,PPI score,ISOA score and otherwise of the two groups before treatment(P>0.05).This shows that there was no difference between the two groups,and they were comparable.2.Comparison of VAS scores,PPI scores and ISOA scores before and after treatment in both groups:obvious diversity can be found in the above three scales before and after treatment in the foot yin meridian Shu points group and foot yang meridian Shu points group(P<0.01),indicating that acupuncture of foot yin meridian Shu points and foot yang meridian Shu points can reduced joint pain and improved range of motion.3.Contrasting differential value of VAS scores,PPI scores and ISOA scores prior and after treatment between the two groups: the differential value of the above three scales prior and after treatment in the foot yang meridian Shu points group was significantly different from that in the foot yin meridian Shu points group(P<0.01),indicating that acupuncture of foot yang meridian Shu points was better than that of foot yin meridian Shu points in reducing the patient’s pain level,increasing joint mobility,and improving the ability of daily living.4.The total effective rate of the foot yang meridian Shu points group was 90.00%、the healing and revealing efficiency rate was67%,and the total effective rate of the foot yin meridian Shu points group was80.00%、the healing and revealing efficiency rate was40%,indicating that the clinical efficacy of acupuncture foot yang meridian Shu points in the treatment of KOA with wind-cold-dampness arthralgia type was better than that of the foot yin meridian Shu points.Conclusion:The Shu point has the effect of dispelling wind and channeling,dislodging cold and alleviate pain,and can be uesd in the treatment of KOA with wind-cold-dampness arthralgia type.Besides,the clinical efficacy of acupuncture of foot yang meridian Shu points in the treatment of KOA with wind-cold-dampness arthralgia type is significantly better than that of foot yin meridian Shu points,so as to find out more effective acupuncture prescriptions for the treatment of KOA with wind-cold-dampness arthralgia type,and improve clinical efficacy.
