Applying External Locking Plates Combined With Xugu Huoxue Decotion In Treatment Of Complex Tibial Open Fractures:a Clinical Research

Objective: To observe,analyze and explore the clinical efficacy of bone revitalizing decoction combined with locking steel plate external therapy for patients with complex tibial open fractures,and to discuss the theoretical basis and development prospect of integrated traditional Chinese and Western medicine in the treatment of open fractures,so as to provide a basis for clinical application.Methods: Patients with open tibial fractures who visited the Department of Trauma and Orthopedics of the Affiliated Hospital of Jiangxi University of Chinese Medicine from September 2015 to January2022 were reviewed.A total of 50 patients were divided into two groups according to the random number table method.A control group was set up to lock the plate for external treatment;The observation group continued bone and blood decoction combined with locked steel plate external treatment group,each group of 25 patients.Debridement,fixation,postoperative rehabilitation and other treatments in all two groups were performed by the same group of medical staff.Anticoagulant,anti-infective,and analgesic drugs are routinely used postoperatively.Patients in the observation group began to take oral bone revitalization decoction plus and minus from the second day after surgery.The fracture healing time and VAS scores of the two groups before treatment,2 weeks after treatment and 4 weeks after treatment were compared.Biochemical indexes such as D-dimer,PT,and APTT were tested after surgery,7 days after surgery and 14 days after surgery.The overall efficacy of TCM was measured after 12 weeks.The condition of the affected limb was observed daily during ward rounds when hospitalized,and observed at follow-up after discharge(follow-up time was three months postoperative,six months postoperative,and one year postoperative,respectively),Johner & Wruhs evaluated the score at the last followup(one year postoperative),and finally recorded,compared and analyzed.Results: All 50 patients completed one-year follow-up.There were no general data such as gender,age,fracture classification,fracture location,and associated injuries in the control group and the observation group(P>0.05).There were no significant differences in VAS score,D-dimer,PT after surgery and one week after surgery(P>0.05),and there were significant differences in fracture healing time,overall efficacy of traditional Chinese medicine,APTT,Johner& Wruhs scores(P<0.05).Among them,the fracture healing time observation group was shorter than that of the control group;The overall efficacy of TCM was cured + improved,and the observation group was more than that of the control group;The observation group was longer than the control group one week and two weeks after APTT index.The Johner & Wruhs score was higher in the observation group than in the control group.Conclusion: Statistical data comparing and analyzing the short-term,medium-and long-term indicators of patients taking Xugu Huoxue Decoction showed that the efficacy of Xugu Huoxue Decoction combined with external locking steel plate in the treatment of complex tibial open fractures can promote blood flow and improve local blood supply in the short term.In the medium and long term,it can promote fracture healing and recovery of limb function of patients,and is a safe and effective treatment.
