Limited Open Reduction And Internal Fixation Combined With External Fixation Tibial Pilon Fractures

Objective: Pilon fracture of tibial is intraarticular fracture . Difficulty of treating is complication and fixation. It involve a significant portion of weight bearing articular surface and overlying metaphysic are notoriously difficult to treat. They ocular when the talus is driven up into the tibial articular surface . All of these give patients its choice of fixation means and prevention and cure of complication. This research reviewed and studied Chinese and Western medical about pilon fracture presented the treatment that used limited open reduction and internal fixation with external fixation to cure the fracture. Chinese medicine curing the bone fracture have got the long history, and at drcreasing swelling to relieve pain, romoting the bone fracture's concrescence have already obtained the good curative effect. Chinese medicine can decrease swelling to relieve pain in the bone fracture earlier period, prevent and cure compartment syndrome, in the medium-term and terminal can promote the bone fracture concrescence.Method: This text established clinic observational tables for 34 cases of open tibial and fibular fracture . All case were divided into cure group and control group (each group had 17 cases)at random .The cure group(male 11 cases; female 6 cases average age 30.7±10.0 years) used limited open reduction and internal fixation with external fixation and control group (male 9 cases; female 8 cases average age 31.5±9.8 years)only used tradition open reduction and internal fixation. We researched and recorded the date of pain, tumidness and bone fracture concrescence.Result: The total efficiency rates of two groups both are 100%; the cure rate of cure group is 81% , which of control group is 72%. Through statistic disposal, the total efficiency rate' s comparison of two groups is P>0.05, and no significant difference. The cure rate' s comparison of two groups is X~2 = 4. 372, P = 0. 041 < 0. 05, so which expresses that the curative effect of cure group is better than that of control group after 3 months. The points' s comparision of pain, tumidness, callus growth before and after the cure significant difference, which expresses that the curative effect of cure group is better than that of control group.Conclusion: limited open reduction and internal fixation with external fixation is a effective fixation means of curing pilon fracture . It could raise cure rate and degrade complication.
