Development And Reliability And Validity Test Of An Evaluation Tool To Match The Supply And Demand Of Community Care Services For The Elderly In The Context Of "Intergration Of Medical Treatment And Care"

Objective: The purpose of this study is to develop a matching tool for the supply and demand of community care services for the elderly in the context of "Integration of Medical Treatment and Care" in China and to test the reliability of the tool.Methods:1.Based on ERG theory,this study builds a framework for matching the supply and demand of community care services for the elderly under the background of "Integration of Medical Treatment and Care".2.The article pool of the evaluation tool is constructed according to the results of the literature study,and the interview results of the semi-structured interview study are summarized and summarized to verify that the entries in each dimension need to be supplemented.3.Using Delphi method to conduct expert correspondence,experts are invited to evaluate the importance of the evaluation tool at all levels of the entry,to form a "Integration of Medical Treatment and Care" background in the elderly community care service supply and demand matching evaluation tool.4.Combined with the Person-Environment Fit connotation transformation questionnaire,elderly demand questionnaire,and community nurse service questionnaire,a uniform measurement scale5.Prior to the official survey,in October 2022,a convenient sampling method was used to survey the readability of the questionnaire for seniors and community nurses attending 33 Community Health Service Stations in Shihezi and 24 Community Health Service Stations.6.From October 2022 to January 2023,a survey of 168 community nurses and 170 community seniors in Shihezi,Xinjiang,was conducted using questionnaire star software.Based on the survey data,the reliability of the questionnaire was tested to form the final version of the community care service supply and demand matching evaluation tool for the elderly in the context of "Integration of Medical Treatment and Care."Results:1.After a review of the literature,a pool of evaluation tool entries was initially formed.2.The study interviewed 17 community seniors and 7 community nurses and formed five elements of basic medical care services,public health services,psychosocial support services,geriatric volunteerism and health guidance services.Taking into account the results of literature studies,the evaluation tools developed by the thematic groups included 3 first-level indicators,7 second-level indicators,and 22 third-level indicators.3.The effective recovery rate of the 15 experts in both rounds of the questionnaire was 88.2% and 100.0%respectively,while the expert authority factor was 0.835 and the value of 0.850,Kendall ’s W was 0.205 and 0.230 respectively.Four three-level entries were deleted,four secondary indicators,one tertiary indicator was modified,and one tertiary indicator was added.The preliminary version scale resulting from the expert queries included three primary indicators,seven secondary indicators and 19 tertiary indicators.4.The content validity index for each entry is 0.857 ～ 1.000 and the total content validity of the scale is0.985.(1)Community Seniors Questionnaire: Project Analysis:(1)The correlation coefficient between all entries and the total score is > 0.3(P < 0.05);(2)All entries CR > 3(P < 0.05).Construct validity: The KMO value is 0.892,the common factor of 5 eigenvalues > 1 is extracted,and the cumulative variance contribution rate is 71.746%.Reliability analysis: The total Cronbach’s α coefficient is 0.943;The Spearman-Brown confidence is 0.736;Retest confidence is 0.950.(2)Community Nurse Questionnaire: Project Analysis:(1)The correlation coefficient between all entries and the total score is > 0.3(P < 0.05);(2)All entries CR > 3(P < 0.05).Construct validity: The KMO value is 0.897,the common factor of 5 eigenvalues > 1 is extracted,and the cumulative variance contribution rate is 63.433%.Reliability analysis: The total Cronbach’s α coefficient is 0.927;The Spearman-Brown confidence is 0.813;Retest confidence is 0.919.Conclusion: The final questionnaire of the study consisted of 28 items in 5 dimensions for daily life care services(2 items);Basic medical care services(11 items);Public health services(4 items);Social support services(8 items);Developmental services(4 items).The reliability and validity of the questionnaire is good,and it has good scientific and feasibility to assess the supply and demand matching of community care services for the elderly.
