Clinical Study On Reduce Filthy Enema Formula In The Treatment Of Refractory Gout With Stasis Heat Block

Objective: To treat patients with refractory gout of stasis heat block and renal function injury by reduce filthy enema formula,to evaluate the clinical efficacy and safety,and to explore a new treatment method and drug for the treatment of this disease.Methods: 72 patients with refractory gout of stasis heat block were randomly divided into treatment group and control group,with 36 patients in each group.The control group with oral febulustat tablets combined with topical 0.9% sodium chloride injection(enema),the treatment group with oral febuxostat tablets combined with topical turbidity enema(enema),the course of a week,if severe pain is unbearable,relying on the anti-inflammatory pain(pain),both groups of cases are no other intervention efficacy observation drugs.Observation of clinical efficacy: TCM syndrome score,visual analog score method(VAS)recorded pain score,erythrocyte sedimentation rate(ESR),and C reactive protein(CRP),blood uric acid(UA),glomerular filtration rate(GFR),cystatin C(Cys C),creatinine(Cr),adverse reactions,and other data were analyzed by SPSS26.0statistical software.Results: 1.Comparison of clinical efficacy of TCM: After the end of 1 week of treatment,the total response rate of the two groups was compared with 93.33% in the treatment group and 68.75% in the control group,and the treatment group was significantly better than the control group(P<0.05).2.The stasis heat block single integral comparison: two groups of subjects after regular treatment symptoms are relieved(P<0.05),treatment group in improving joint redness,heat pain,activity,thirst,urine yellow is better than the control group(P<0.05),and two groups before and after the treatment group control in improving tophi,skin color purple dark have little effect(P> 0.05).3.Comparison of VAS scores: The VAS scores of the two groups decreased compared with before treatment(P<0.05),and the treatment group was more effective before and after treatment(P<0.05).4.Comparison of efficacy indicators: The levels of ESR,CRP,UA,GFR,Cr,and Cys C in the two groups were improved compared with those before treatment(P<0.05),and the treatment group was better than the control group(P<0.05).5.Analysis of concomitant medication: the subjects in both groups took anti-inflammatory analgesia during treatment,among which,5patients in the treatment group,the concomitant medication rate was16.67%,6patients in the control group,and the concomitant medication rate was 18.75%.There was no difference in the combined medication between the two groups(P>0.05).6.Drug safety comparison: the safety indicators between the two groups(P>0.05),and after the treatment group red cell count and valley grass transaminase,valley gamma aminase decreased than before treatment(P<0.05),but fluctuates within the normal value range,the rest of the index comparison suggest no statistical difference(P>0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion: 1.The turbidity perfusion square combined febustat treatment of stasis heat block refractory gout patients reflects the good curative effect,improve clinical symptoms and signs can reduce uric acid,creatinine,cystatin C,improve glomerular filtration rate,significantly inhibit ESR and CRP inflammatory indicators,reduce TCM syndrome score,play to the advantages of traditional Chinese medicine,for the treatment of this disease to explore a new treatment and drugs.2.It is safe and effective in the treatment of blood stasis and heat block refractory gout,which is worthy of clinical application and promotion.
