| Objective:To observe the effect of abdominal acupuncture combined with thunder and fire moxibustion on seventeen vertebra and oral ibuprofen sustained release capsules in the treatment of primary dysmenorrhea with cold coagulation and blood stasis in improving clinical symptoms and on the blood flow dynamics of uterine artery,so as to seek a good effect and no obvious side effects for clinical method.Methods:A total of 64 PD patients with cold coagulation and blood stasis who visited the acupuncture Department of the Second People’s Hospital Affiliated to Fujian University of Chinese Medicine and were enrolled in Fujian University of Chinese Medicine from June 2022 to September 2022 were randomly divided into two groups.The observation group was treated with abdominal acupuncture combined with thunderfire moxibustion seventeen vertebrae,starting 5 days before menstruation,once a day;The control group took ibuprofen sustained-release capsules orally,0.3g/ time,twice a day,from 3 days before menstrual period until the second day of withdrawal.Both groups were treated continuously for 5 days per menstrual cycle and for 3 menstrual cycles(1 course of treatment).Visual analog scale(VAS),TCM syndrome score,dysmenorrhea symptom score,uterine artery pulsation index(PI),resistance index(RI),systolic to diastolic peak flow rate ratio(S/D)were recorded before and after treatment.VAS and dysmenorrhea symptom score were followed up 3 months after treatment,and statistical analysis was performed.Results:1.A total of 64 patients with primary dysmenorrhea of cold coagulation and blood stasis type were included in this study,and 61 of them were finally studied(2 cases were excluded from the observation group and 1 case from the control group,with a total shedding rate of 4.69%).2.Baseline:Before treatment,there were no statistically significant differences in age,age of menarche,course of disease,menstrual cycle,degree of dysmenorrhea,VAS,TCM syndrome score,dysmenorrhea symptom score,PI,RI and S/D between the two groups(P>0.05),indicating comparability.3.VAS:(1)After treatment: compared with before treatment,both groups decreased(P<0.05);There was no difference between groups(P>0.05).(2)At follow-up: compared with the group before treatment,both groups decreased(P<0.05);There was no difference in the observation group(P>0.05),but there was a difference in the control group(P<0.05).The improvement of observation group was better than control group(P<0.05).4.TCM syndrome scores:After treatment,the scores of both groups were decreased(P<0.05).Compared with each other,observation group was lower than control group(P<0.05).5.Dysmenorrhea symptom score:(1)After treatment,the scores of both groups were decreased by intra-group comparison(P<0.05);There was no difference between groups(P>0.05).(2)At follow-up: compared with the group before treatment,both groups decreased(P<0.05);There was no difference in the observation group(P>0.05),but there was a difference in the control group(P<0.05).The improvement of observation group was better than control group(P<0.05).6.Uterine artery blood flow dynamics(PI,RI,S/D): After treatment,compared with before treatment in the same group,all decreased(P<0.05).The reduction degree of observation group was better than control group(P<0.05).7.Clinical efficacy: The short-term total effective rate was 93.33% in the observation group and 87.10% in the control group,with no statistical significance(P>0.05).Long-term total effective rate: 83.33% in the observation group and 51.61% in the control group,the difference was statistically significant(P<0.01).Conclusions:1.Abdominal acupuncture combined with thunity-fire moxibustion Seventeen vertebra and ibuprofen sustained release capsules can effectively improve the clinical symptoms and the related values of uterine arterial blood flow dynamics in patients with primary dysmenorrhea of cold coagulation and blood stasis type,but the former can improve the traditional Chinese medicine syndrome and the related values of uterine arterial blood flow dynamics better.2.The short-term efficacy of abdominal acupuncture combined with thunderfire moxibustion in Seventeen vertebra is similar to that of ibuen sustained release capsule in the treatment of primary dysmenorrhea of cold coagulation and blood stasis type,but the long-term efficacy of the former is better,suggesting that abdominal acupuncture combined with thunderfire moxibustion in seventeen vertebra has more lasting effect. |
