ObjectiveTo observe the clinical effect of warm acupuncture and moxibustion on 17 vertebrae combined with acupuncture on Dazhui and Geshu in the treatment of primary dysmenorrhea.Methods66 patients with primary dysmenorrhea were randomly divided into treatment group and control group(33 cases in each group).The treatment group was treated with warm acupuncture and moxibustion of 17 vertebrae + acupuncture of Dazhui and Geshu,while the control group was treated with oral fenbide.Observe and record the changes of TCM dysmenorrhea symptom total score,visual analogue score(VAS)and serum estradiol(E2)before and after the treatment of three menstrual cycles in the treatment group and the control group,and follow-up record TCM dysmenorrhea symptom total score and VAS score three months after the end of the treatment.Use statistical analysis to observe the clinical effect of the treatment of primary dysmenorrhea of cold blood stasis type by Warming Acupuncture and seventeen vertebrae.Result1.There was no significant difference in age,course of disease,dysmenorrhea symptom score and severity of dysmenorrhea between the two groups before treatment(P > 0.05).2.Clinical effect comparison: the total effective rate of the observation group is90.32% in the near future,and the total effective rate of the control group is 86.66%.There is no significant difference between the two groups(P>0.05),indicating that the short-term effect of the two groups is similar.The long-term total effective rate of the follow-up observation group was 87.10%,and the long-term total effective rate of the control group was 43.33%,P<0.05,the difference was statistically significant,indicating that the long-term effect of the observation group was better than that of the control group.3.Comparison of TCM dysmenorrhea symptom score and VAS score: after treatment,the symptom score and VAS score of the two groups were lower than before.There was no significant difference between the two groups by rank sum test,indicating that there was no significant difference between the two groups in improving dysmenorrhea symptom score and VAS score.The improvement of dysmenorrhea symptom score and VAS score in the follow-up treatment group was better than that in the control group(P<0.05).4.Comparison of serum estradiol: after treatment,the level of serum estradiol in the treatment group was significantly lower than that before treatment,the difference was statistically significant(P<0.05),while the level of serum estradiol in the control group was not significantly lower(P>0.05),the difference was not statistically significant.5.Analysis of clinical safety: under the current experimental conditions,no adverse reactions were found in the treatment of warm acupuncture and moxibustion mainly combined with acupuncture of Dazhui and Geshu.Conclusion1.Clinical observation shows that the short-term effect of the two treatment schemes is close,and both of them can effectively improve the pain symptoms of patients,but the long-term effect of the treatment group is more stable and lasting.Dazhui and Geshu can strengthen the effect of warming the meridians and dispersing the cold,regulating the meridians and alleviating pain,promoting blood circulation and removing blood stasis.At the same time,the method of warming acupuncture before the meridians can also enhance the Yang Qi of human body,play the role of treating both the specimen and the blood stasis,and further enhance the long-term effect of warming acupuncture.2.Warm acupuncture and moxibustion of 17 vertebrae combined with acupuncture of Dazhui and Geshu can effectively reduce the E2 level of the subjects,which is better than that of the control group.It is speculated that the mechanism of reducing E2 level may reduce the promoting effect on prostaglandins,so as to relieve dysmenorrhea symptoms.
