Effect Of Anzhong Decoction On Upper Abdominal Pain Syndrome With Disharmony Between Liver And Stomachand Influence Of Gastrin-17

Objective:To observe the improvement of Anzhong Decoction on clinical symptoms,quality of life,anxiety,depression and recurrence rate in patients with epigastric pain syndrome(EPS),evaluate the clinical efficacy of Anzhong Decoction and observe the effect of Anzhong decoction on serum gastrin-17 content,so as to explore its possible mechanism of action.Methods:1.A total of 72 cases of liver and stomach disordered EPS patients admitted to the outpatient department of spleen and stomach Diseases of our hospital from February 2022 to December 2022 were collected and divided into treatment group and control group with 36 cases respectively by random number table method.The treatment group was given Anzhong Decoction and the control group was given Rabeprazole sodium enteric-coated tablets for a course of 4 weeks.2.Clinical data such as age,gender and course of disease were collected.Chinese medicine symptom quantification scale,Short form-Nepean dyspepsia index(SF-NDI)scale score,Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS)score before and after treatment were recorded in the two groups.The serum gastrin-17 content of the two groups before and after treatment was detected.Follow-up was conducted 4 weeks after drug withdrawal to observe and compare the recurrence of the two groups.Results:1.Comparison of clinical data:There was no statistically significant difference in age,gender and course of disease between the two groups(P>0.05),which was comparable.2.Comparison of TCM Syndrome efficacy:The total effective rate of treatment group was better than that of the control group(87.88%vs73.53%,P<0.05).3.Comparison of the total scores of TCM syndromes:After treatment,the total scores of TCM syndromes in both groups decreased compared with before(P<0.05),and the improvement of TCM syndromes in the treatment group was better than that in the control group(P<0.05).4.The integral analysis of single symptom of traditional Chinese medicine: after treatment,the integral of each single symptom of both groups decreased compared with before(P<0.05).The treatment group had an advantage over the control group in the aspects of upper abdominal pain,distension of both sides,attack or aggravation due to emotional distress,belching and upset(P<0.05),but there was no significant difference between the treatment group and the control group in the aspects of upper abdominal burning pain and acid regurgitation(P>0.05).5.Analysis of quality of life efficacy:After treatment,concinipine dyspepsia index(SFNDI)scores in 2 groups were improved compared with before(P<0.05),and the improvement of scores in the treatment group was better than that in the control group(P<0.05).6.Comparison of self-rating scales for anxiety and depression:After treatment,SAS and SDS scores of the two groups were decreased compared with before(P<0.05),and the improvement of SAS and SDS scores of the treatment group was better than that of the control group(P<0.05).7.Recurrence rate analysis:4 weeks after the end of treatment,the recurrence rate of the treatment group was lower than the control group(17.24%vs44%,P<0.05).8.Correlation analysis: The serum gastrin-17 content in patients with hepato-gastric disharmony EPS was positively correlated with the total scores of TCM syndroms and SAS and SDS scores(P<0.05);The total scores of TCM syndromes were positively correlated with SAS and SDS scores(P<0.05).9.Comparison of serum gastrin-17 content: The serum gastrin-17 content of the two groups was changed after treatment(P<0.05).conclusion:1.Anzhong Decoction can improve the clinical symptoms,anxiety and depression,improve the quality of life and reduce the recurrence rate of patients with ESP with liver and stomach discompatibility,and has no obvious adverse reactions,which is better than Rabeprazole sodium enteric-coated tablets.2.After the treatment of Anzhong Decoction,the serum G-17 level was decreased,and it was speculated that it might treat the disease by regulating the level of related gastrointestinal hormone,and the change of serum G-17 level was related to the level of clinical symptoms,anxiety and depression.
