| Objective: The purpose of this study is to construct an evaluation index system for the core competencies of dementia nurses,and to provide corresponding references for the training and certification of dementia nurses in the future.Methods: This study was divided into two stages: the construction of the first draft of the index system and the formation of the final draft.The stage of the construction of the first draft of the index system: 1.The literature related to the core competence of dementia specialist nurses at home and abroad was searched by literature analysis,and the relevant indexes were extracted by content analysis.2.Based on the results of literature analysis,the interview outline was drawn up combined with the purpose of constructing the index system,the selected experts were interviewed,and the index extracted by literature analysis was combined with the data obtained from the interview to form an item pool.After several group discussions,the indicators were adjusted,deleted and merged,and the core competence evaluation indicator system of dementia specialist nurses was initially constructed.The stage of the formation of the final draft of the index system: 1.Delphi method was used to conduct two rounds of expert consultation to solicit experts’ suggestions on the construction of the evaluation index system of the core competence of dementia specialist nurses,further revise the indicators,and Excel 2019 and SPSS 25.0 software were used to collect,collate and analyze the data.2.The analytic hierarchy process was used to calculate the weight value of each index and check the consistency,so as to form the evaluation index system of core competence of dementia nurses.Results: The construction of the first draft of the index system:(1)A total of 14 related literatures were included in this study,including 10 in English and 4 in Chinese.9 main indicators were extracted.They were ethics and law,promotion of patients’ activities of daily life,cooperation with informal caregivers or professionals,support for informal caregivers,management of behavioral and psychological symptoms of dementia,effective communication,people-oriented,understanding of dementia and related knowledge and palliative treatment.(2)A total of 5 experts were interviewed in this study.According to the analysis of the interview data,7 topics and 17 sub-topics were extracted.The first draft of the index system was formed by integrating with the results of literature analysis,including 7 first-level indicators,21 second-level indicators and 87third-level indicators.Part of the final draft of the indicator system:(1)A total of 19 experts participated in two rounds of expert consultation,and the recovery rates of the two rounds of questionnaires were 100% and 89.47% respectively,indicating that the enthusiasm of experts to participate in this study was high;the authority coefficients of the two rounds of experts were 0.88 and 0.89 respectively.The harmony coefficients of the first index,the second index and the third index were 0.178,0.184 and 0.118,respectively.(2)By using the analytic hierarchy process,the weight values of the primary index were as follows: the ability of observation and evaluation was 0.234,the ability of care and symptom management of patients with dementia was 0.234,the ability of effective communication was 0.167,the ability of education and support was 0.115,the ability of ethical and legal practice was 0.115,the ability of evidence-based nursing and scientific research innovation was 0.077,and the ability of cooperation was 0.059.The weight range of the combination of secondary indicators was 0.008 to 0.067.The weight range of the three-level index combination was 0.003 to 0.038.The CR values of the consistency test results of all levels of indicators were less than 0.1.This study finally formed 7 first-level indicators of observation and evaluation ability,effective communication ability,care and symptom management ability,cooperation ability,education and support ability,ethics and legal practice ability,evidence-based nursing and scientific research innovation ability.The evaluation index system of core competence of dementia specialist nurses was composed of 21 second-level indicators and 96 third-level indicators.Conclusion: In this study,the evaluation index system of core competence of dementia specialist nurses is constructed by combining literature analysis,interview method,Delphi method and analytic hierarchy process,including 7 first-level indexes,21second-level indexes and 96 third-level indexes.And the corresponding weight is established for all levels of indicators,and the structure is reasonable.Strict quality control has been carried out in the construction and revision of the index system,which has a certain feasibility and practical significance,and can comprehensively evaluate the core competence of dementia specialist nurses.It provides a certain reference basis for the training and certification of dementia specialist nurses in China in the future. |
