ObjectiveThe purpose of this research was to build up a evaluation system of core competencies for intervention specialized nurses and served as basis and standards of the employment, training, evaluation of specialized nurses in our country. The second step was to investigate the intervention of nursing core competence status of nurses-theoretical knowledge ability, to learn about the theoretical knowledge ability of core competences and the present situation of training in intervention specialized nurses.MethodThe research included two parts.The first part was the literature investigation, experts interview and theoretical analysis, through which the intervention specialized nurses’ core competence evaluation system model was constructed. By using Delphi method and with the assistance of questionnaires, two rounds of consultation with the experts analysis of the index weight coefficient and development of a formal intervention specialized nurses’ core competence evaluation system were performed. Through calculating experts’ positive coefficient, authority coefficient, variation coefficient, coordination coefficient and related statistical indicators, the credibility and representative of the Delphi method were validated. In the second part, the evaluation tools of theoretical knowledge ability of the core competence evaluation system was designed. The Delphi method was used again to form the specialized theory knowledge to master outline. According to the principles of proposition design test paper and using random sampling method,120 nurses of 3 class hospital in Changsha were surveyed in the form of written examination. Through data analysis, the theoretical knowledge ability, cultivation situation and related influencing factors were accumulated. Commercial software of Excel2007, SPSS 17.0 was used to analyze the experimental results. Statistical methods used were measurement data using mean, standard, and t-test, Count data adoption rate and composition and χ2 test, P< 0.05 for the difference was statistically significant.Results1. The core competence system was composed of 7 first-grade indicators which was theoretical knowledge, practical ability, critical thinking skills, communication skills, clinical nursing management ability, scientific research ability, counseling and guidance ability of experts respectively,3 second level indexes and 56 third level indexes. The positive coefficient of two rounds of experts consultation were 100%,88.89%; the coefficient of authority of experts consultation was 0.90, the judge coefficient was 0.92, the familiar degree was 0.88, the coordination coefficient of each index were 0.357,0.401,0.436,chi square test, P<0.05, which has statistical significance.Experts’views of core competencies evaluation index of all levels of nurse specialist at interventional department were intensive, the results have high reliability.2.By two rounds of experts consultation an outline of specialized theories of intervention which needs to be grasped was formed. The coefficient of authority of expert consultation was 0.89,the judge coefficient was 0.89, the familiarity was 0.89, all those index>0.7, which shows the High reliability of the consultation results. By pre surveying 35 nurses doing intervention care,we found that the difficulty coefficient was 0.65, the difficulty coefficient of each section were respectively 0.69,0.81,0.50,0.58,0.69, (>0.4), which were good; the differentiation degree was 0.36, the discrimination of every section were 0.36, 0.28,0.40,0.37,0.39 respectively (>0.20), the different degree was good; reliabilitycoefficient was 0.826,> 0.70, which means that the results were reliable. Each data index shows the good quality of the questionnaire and its feasibility to be used as a tool to evaluate the intervention specialist nurses’ master degree of core speculative knowledge.In the survey of 120 nurses, the intervention specialist nurses who ows a certification were 45 (37.5%), noncertified nurses,75 (62.5%). The mean score was 72.09± 10.72, people who disqualified in specialist group were 3(6.67%),42(93.33%) passed it, the average score was 74.76±9.78; 18(24%) failed in non specialist nurses group, there were 57 (76%)who passed it, the average score was 69.41 ±11.65. the average total score in specialist nurses group were higher than non specialist nurses group, which has statistically significance(P<0.01). In the first part of interventional therapy and nursing knowledge, anatomy knowledge of the second part,radiology knowledge scores of the third part,the score of the specialist nurses group were higher than the non specialist nurses group, the difference has statistically significant (P<0.05), Scores in the ethics knowledge of the fourth part and the psychology knowledge in the fifth part had no significant difference (P>0.05). Training time in the intervention department mainly last 1-3 days, the most of nurses were on-the-job-training, including interventional therapy and nursing related knowledge of anatomy, knowledge of radiology.Scores of the group the nurses in the single factor analysis, the title, educational background of some knowledge scores were affected, and trained specialist working time and number of basic no effect of scoring, there was no statistically significant difference (P>0.05).Conclusion1.The evaluation system was build, which includes 7 first level index, 23 second level index and 56 third level indexes of core competencies about specialist nurse, it was reliabled.2.Nurses engaged in the interventional department was not receiving comparatively short and infrequent training, and the contents and methods of training was not comprehensively enough.We should improve the training level,promote the development of interventional nurses.
