Objective: To investigate the correlation between monocyte-high-density lipoprotein ratio(MHR)and neutrophil count-lymphocyte count ratio(NLR)and cerebral small vessel disease(CSVD).Methods: A retrospective study was conducted to collect 77 CSVD patients from the Department of Neurology,Shengjing Hospital,China Medical University from September 2020 to January 2023,as the case group.After matching the gender and age,a total of 77 cases of the health examination population in our hospital during the same period were collected as the control group.The general conditions and serological indicators of the two groups were collected.MHR values were calculated according to monocytes count and high-density lipoprotein,NLR values were calculated according to neutrophil count,lymphocyte count,and grouped according to clinical manifestations and MRI diffusion function imaging of the head.The case group was further divided into mild to moderate group and severe group according to CSVD stress score,and WMH was divided into light or no group and moderate to severe group according to Fazekas score.Statistical analysis of general data such as MHR and NLR.Results: Compared with the control group,there were no significant differences in age,sex,smoking,alcohol consumption,BMI,LYM,PLT,TC,TG,LDL and HCY in the CSVD group(P>0.05).The CSVD group had higher levels of hypertension(61.0%)and diabetes mellitus(49.4%)than those in the control group,WBC [6.50(5.35-7.88)],NEU[3.94(3.21-5.04)] were higher than those in the control group,HDL [1.01(0.87-1.31)] were lower than those in the control group,and MHR values [0.43(0.29-0.57)] were higher than those in the control group [0.31(0.22-0.41)],NLR value level[2.09(1.69-3.41)] was higher than that of the control group [1.76(1.30-2.40)],and the difference was statistically significant(P<0.05).There was also a statistically significant difference in MON levels between the two groups(P<0.05).Through binary logistic regression analysis,the proportion of hypertension and diabetes,MHR values and NLR values in the CSVD group were still different from those in the control group,and the differences were statistically significant(P<0.05).Through ROC curve analysis,the area under the curve(AUC)of MHR was 0.672 [95% CI(0.587,0.757),P<0.05],sensitivity and specificity were 47% and 86%,respectively,and the area under the curve(AUC)of NLR was 0.670 [95% CI(0.585,0.754),P<0.05],sensitivity and specificity were 39% and 87%,respectively.Compared with the mild to moderate CSVD group,men(75.0%),hypertension(79.2%)accounted for higher levels,NEU [4.54(3.78-5.59)],HCY[14.58(10.71-19.98)],NLR [3.17(2.04-3.89)] are higher and LYM(1.57±0.60)had lower mean values in Severe CSVD group,the differences were statistically significant(P<0.05).Binary Logistic regression analysis was performed in the proportion of men with severe CSVD and higher levels of NLR values,and there were significant differences between the two groups(P<0.05),and there was no significant difference between hypertension and HCY levels between the two groups(P>0.05).Compared with mild or no WMH,patients with moderate to severe WMH had higher hypertension(75.9%),lower levels of LYM(1.50±0.56),glycated hemoglobin [5.50(5.30-6.10)],higher mean values of HCY [13.18(11.14-19.73)],NLR [3.29(1.99-3.99)],and there were significant differences between the two groups(P<0.05).There is no significant difference in MHR between the two groups(P>0.05).By binary logistic regression analysis,the mean glycated hemoglobin in the moderate to severe WMH group was still lower and the NLR value was still higher,and there was a statistically significant difference between the two groups(P<0.05).Conclusion:1.MHR and NLR may be independent risk factors for CSVD,and may have certain predictive value for CSVD.2.NLR may have some predictive value for the severity(image manifestations)of CSVD.3.NLR may have some predictive value for the severity of white matter hyperintensity in CSVD.
