Objective:To analyze the correlation between monocyte count/high density lipoprotein cholesterol ratio and SYNTAX score in patients with unstable angina pectoris, and to explore the predictive value of MHR in the severity of coronary artery disease in patients with unstable angina pectoris. Methods:The clinical data of 249 patients with unstable angina pectoris who were hospitalized in the Department of Cardiovascular Medicine of the first affiliated Hospital of China Medical University from June 2016 to June 2017 and underwent elective percutaneous coronary angiography were analyzed retrospectively.Hypersensitive C-reactive protein?hs-CRP?,blood routine examination?including white blood cell count, neutrophil count, monocyte count, etc.?within 24 hours of admission, fasting blood lipid?including total cholesterol?TC?,low density lipoprotein cholesterol?LDL-C?,high density lipoprotein cholesterol?HDL-C??,liver function and so on were detected.MHR was calculated by the ratio of Mon and HDL-C.According to the results of coronary angiography, patients mentioned above were divided into three groups, including single vessel lesion group?group A?,double vessel lesion group?group B?and three vessel lesion group?group C?.And SYscore was calculated according to the special rules. The difference of MHR?WBC?hs-CRP?SYscore was compared among the three groups. The correlation between MHR and SYscore was analyzed.Results:1.Comparison of general clinical data among groups with different lesion branches:the proportion of diabetic history in group C was significantly higher than that in group A?P=0.000?,but there was no significant difference between group B and other two groups?P>0.05?.There were statistical differences in SYscore among the three groups. The SYscore was the highest in group C,followed by group B and group A.2.Comparison of biochemical Indexes among groups with different pathological branches:WBC in group C was significantly higher than that in group B?P=0.002?,but there was no significant difference between group A and the other two groups?P>0.05?.NE was significantly higher in group C than in the other two groups?P1=0.031,P2=0.000?,whiletheremainingtwogroupshadno difference?P>0.05?.Mon in group C was significantly higher than that in group A,but there was no difference between group B and other groups?P>0.05?.MHR in group C was significantly higher than that in the other two groups?P1=0.001,P2=0.028?,though compared with group A,group B has a tendency to increase, there was no significant difference between them?P>0.05?.3.There was a positive correlation between MHR and hs-CRP,WBC,NE respectively. The correlation coefficients are r1=0.132?P=0.038?,r2=0.485?P=0.000?,r3=0.387?P=0.000?.4.HDL-C was negatively correlated with SYscore, the correlation coefficient is r=-0.148?P=0.019<0.05?;there was a significant positive correlation between SYscore and Mon,WBC,MHR respectively, the correlation coefficients are r1=0.179?P=0.005<0.01?,r2=0.206?P=0.001<0.01?,r3=0.134?P=0.034<0.05?;The correlation between MHR and SYscore was most significant. Conclusion:The ratio of monocyte count/high density lipoprotein cholesterol was closely correlated with the SYNTAX score in patients with unstable angina pectoris. The higher the ratio of monocyte count/high density lipoprotein cholesterol, the higher the SYNTAX score, the more the number and severity of coronary artery lesion, the more complex.
