Objective:Acute myocardial infarction is the highest rate of heart disease at present in our country.Timely and accurate clinical diagnosis is essential.There are some deficiencies in the existing myocardial markers.Therefore,it is urgent to explore new myocardial markers.The main purpose of this study was to establish a diagnostic model by detecting the relative quantitative levels of miRNA in peripheral blood of patients with acute myocardial infarction and healthy people,and to explore the diagnostic value of circulating miRNA levels in patients with acute myocardial infarction.Methods:By screening high-throughput data samples in the database,five mirnas were selected combined with literature.Quantitative PCR was used to detect miRNA expression levels in peripheral blood immediately before PCI.Logistic regression equation was used to combine the indicators to obtain a serum combination diagnostic model,and receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of these mirnas and combination models.Results:A total of 5 mirnas were detected,among which 4 mirnas(miR-199b-5p,let-7b-5p,miR-23a-5p and miR-140-5p)were significantly up-regulated in AMI group,while 1 miRNA(miR-133b)had no significant difference in expression level.ROC curve showed that,Among the five mirnas,miR-199b-5p showed the best diagnostic ability,and its area under the curve(AUC)was 0.840.Logistic regression analysis equation was used to combine the above four significantly differentially expressed mirnas to construct a diagnostic model.The area under the curve of the constructed model was 0.861(95%CI:0.7749-0.9476,specificity:93%,sensitivity:80%).It shows that it has excellent diagnostic ability of acute myocardial infarction.Four differentially expressed mirnas were combined with traditional myocardial markers to construct a diagnostic model.The area under the curve of the model was 0.965(95%CI:0.9312-0.9991,specificity:87.3%,sensitivity:100%),which was significantly higher than the diagnostic ability of traditional myocardial markers.Conclusion:The relative expression level of miR-199b-5p,let-7b-5p,miR-23a5p,miR-140-5p and miR-133b in whole blood of patients with acute myocard ial infarction was significantly up-regulated.The diagnostic model composed o f miR-199b-5p,let-7b-5p,miR-23a-5p,miR-140-5p and miR-133b has excellen t diagnostic ability for acute myocardial infarction.The diagnostic efficacy of miR-199b-5p,let-7b-5p,miR-23a-5p,miR-140-5p and miR-133b combined wit h traditional myocardial markers is higher than that of traditional myocardial markers alone,which has certain clinical significance.
