| Objective: To evaluate the five-year quality of life(QOL)after oral cancer(OC)operation by questionnaire, and analyze the influencing factors of long-term QOL after OC operation. Methods: From January 2015 to June 2017,142 oral cancer(OC)patients who underwent surgery in the Department of Oral and Maxillofacial Surgery of the First Affiliated Hospital of Bengbu Medical College were analyzed retrospectively. The fourth edition of the University of Washington quality of life questionnaire(UW-QOL)was used to investigate the quality of life of patients 5 years after operation. The data were analyzed by SPSS22.0 software to explore the influence of 9 demographic factors and 10 clinical factors on the long-term QOL of OC patients after operation. Results: Univariate analysis showed that age, religious belief, occupation, clinical stage, radiotherapy, neck lymph node dissection, segmental jaw resection, recurrence and comorbidity had different effects on long-term Physical Health-related Quality of Life(PHR-QOL)(P < 0.05).Age,marriage,religious belief, educational level, occupation, family income, family care, clinical stage, radiotherapy and recurrence affected the long-term Social Psychology Health-related Quality of Life(SPHR-QOL)in different degrees(P < 0.05).Multivariate analysis showed that clinical stage and segmental jaw resection were the main influencing factors of long-term PHR-QOL,marriage,family care, religious belief, relapse and clinical stage were the main influencing factors of long-term SPHR-QOL,and clinical stage, relapse, family care and segmental jaw resection were the main factors of long-term average score of UW-QOL.Conclusion:1.The long-term PHR-QOL of OC patients is mainly affected by clinical stage and segmental jaw resection, and the long-term SPHR-QOL is mainly affected by demographic factors such as marriage, family care and religious belief;2.Long-term QOL was mainly affected by clinical stage, recurrence, family care and segmental jaw resection;3.On the premise of guaranteeing the survival rate, keeping the jaw intact as much as possible in treatment, emphasizing the thoroughness of surgical treatment, reducing recurrence, which is helpful to the recovery and adaptation of physical function defect, and a certain degree of religious belief and family support are helpful to the improvement of long-term SPHR-QOL.Through the above measures, the long-term overall QOL of OC patients can be improved. |
