Quality Of Life Of Caregivers Of Patients With Primary Liver Cancer And Its Influencing Factors

ObjectiveTo understand the quality of life(QOL)of primary liver cancer(PLC)patients and analyzed its influencing factors.Analyse the caregivers' ture feelings in-depth.To provide theoretical basis for clinical nursing work to draw up measures in order to improve.Results can provide theoretical basis for taking effective interventions to improve the caregivers' quality of life.MethodsThis research is a descriptive study and used the mixed method of quantitative research combining with qualitative research.1.A cross-section studyA convenience sample of 214 patients with PLC and their caregivers were investigated with the questionnaire about genera condition and the WHOQOL-BRIEF,Symptom Checklist 90(SCL-90)and the Social Support Rating Scale(SSRS).By using SPSS21.0 software to make data analysis.2.Qualitative researchThe research adopts the purpose sample method to select 12 femalespouse caregivers of patients with PLC,interview them used the phenomenological method study.The data collected by semi-structural interview and analyzed by Golaizzi analysis method.ResultsQuantitative research part1.The caregivers scored(75.49±10.81)on QOL,single item scores of four dimensions were:physical QOL was(24.04±3.86),mental QOL was(18.55±3.25),social relations was(8.97±3.32),environmental field was(23.94±3.78).2.The scores of SSRS were(31.61±8.14),single item scores of three dimensions were:objective support was(8.17±1.99),subjective support was(18.23±5.89),utilization degree was(5.21±1.98).3.The scores of anxiety was(17.07±4.78),the scores of depression was(23.56±6.32).4.The univariate analysis showed that age of caregivers,residence,relationship with patients,whether live with patients,education level,family income,mode of payment,career,whether to have other caregivers,whether to need caregivers,whether to have illness,metastasis of tumor,patient profession,social support level,care time were related to QOL level.5.The correlation analysis showed that the score of QOLwas positively correlated with the score of social support(r=0.518,P < 0.01),negativelycorrelated with the score of anxiety(r=0.351,P<0.01),negatively correlated with the score of depression(r=0.491,P<0.01).6.Multiple regression analysis show that depression,mode of payment,whether to have other caregivers,whether to have illness,social support entered the regression equation,all of them which could explain40.6% of the quality of life of the caregivers.Qualitative research partThrough analysis and summarize,six main themes were identified.1.expect great treatment.2.patients had great negative effect on femle spouse.3.extremely emotional dependenc.4.complicated negative emotions and psychological experience.5.heavy economy burden.6.weak support.Conclusion1.The scores of QOL was relatively low,mental health also not optimistic.Depression,mode of payment,whether to have other caregivers,whether to have illness,social support of caregivers were influencing factors of QOL.2.The spouse caregivers had abvious negative emotions.It is caused by heavy pressure,expect great treatment and lack of effective support.
