Objective: 1.To understand the relationship and mediating effect between quadriceps muscle strength,motor function,walking ability and daily activity ability in patients with disuse knee contracture.2.To explore the effect of rehabilitation nursing combined with traditional Chinese and Western medicine on the lower extremity functions of knee quadriceps muscle muscle strength joint motion pain degree muscle atrophy knee function movement function gait walking ability and daily activity ability in patients with disuse knee contracture.Methods: 1.126 disused knee contractures were investigated by general data,MMT rating scale,Fugl-Meyer Motor Function Rating Scale,Holden Functional Walking Scale and Barthel Index scale.2.86 patients with disuse knee contracture were randomly divided into conventional rehabilitation nursing group(conventional rehabilitation nursing)and integrated traditional Chinese and western medicine rehabilitation nursing group(conventional rehabilitation nursing + acupoint nursing along meridians(namely along the patient’s foot Yangming stomach meridian and foot Taiyin spleen meridian 2 meridian direction and acupoint on the manipulation of vibration and massage)+ knee exercise training),with 43 cases in each group.Before intervention and 2 weeks after intervention,quadriceps muscle strength,joint motion,pain,muscle atrophy,knee function,motor function,gfunction ait and walking ability,and daily activity ability of patients were evaluated.Results: 1.The quadriceps muscle strength score of disused knee contracture patients was3.00(1.00,4.00)points;Motor scores were 14.00(9.00,24.00),and the lowest scores were flexor synergism and hyperreflexes.Walking ability score was 1.00(0.00,4.00)points;The daily activity ability score was 52.50(25.00,80.00),and the two lowest scores were bathing and climbing stairs.2.Spearman rank correlation analysis showed that quadriceps muscle strength,motor function,walking ability and daily activity ability were positively correlated(P <0.001).3.The mediating effect analysis showed that quadriceps muscle strength and walking ability had a chain mediating effect on motor function and daily activity ability.4.Influence on functional recovery of lower limb knee: The comparison of quadriceps muscle strength,joint flexion range of motion,VAS score,thigh circumference difference,Lysholm knee score and FMA motor function score between the two groups was statistically significant(P < 0.05).5.Influence on gait: There were statistically significant differences between the two groups in the comparison of starting,foot lifting height,stride length,gait compatibility,stride continuity,walking path,trunk stability and stride width(P < 0.05).6.Influence on walking ability and daily activity ability: There were statistically significant differences in Holden Functional Walking Scale and Barthel Index scale scores between the two groups(P < 0.05).Conclusion: 1.The quadriceps muscle strength,motor function,walking ability and daily activity ability of patients with disuse knee contracture need to be further improved,and the four are closely related and influence each other.Quadriceps muscle strength and walking ability play a chain mediating role between motor function and daily activity ability.2.Conventional rehabilitation nursing and rehabilitation nursing intervention combined with traditional Chinese and western medicine can improve the quadriceps muscle strength,joint motion,knee joint function,motor function,walking ability and daily activity ability of patients,reduce pain and muscle atrophy degree,improve gait,and the effect of rehabilitation nursing intervention combined with traditional Chinese and western medicine is better.
