Objective: To analyze the clinical and ultrasonographic features of appendix tumors in order to improve the ultrasonographic diagnosis of appendix tumors.Methods:Retrospective analysis of clinical data(such as age,sex,duration of disease,blood routine,white blood cell count,carcinoembryonic antigen,etc.)and characteristics of appendix sonogram of 80 cases of appendiceal tumors(64 cases of mucinous tumors,10 cases of adenocarcinoma,6 cases of neuroendocrine tumors)and 100 cases of appendicitis(46cases of acute suppurative appendicitis,33 cases of acute simple appendicitis,12 cases of acute gangrenous appendicitis,9 cases of periappendiceal abscess)(For example,the outer diameter of the appendix,the wall thickness,whether the wall is accompanied by nodules,whether the cavity is accompanied by fecal stones,whether the peripheral mesangium is thickened,whether it is accompanied by peripheral hydrops and changes in the shape of the appendix,such as "onion skin" changes,"sausage" changes,and "spherical" expansion of the tip,etc).Results: There were statistically significant differences between the two groups in clinical indicators such as age of onset,duration of disease,white blood cells and carcinoembryonic antigen(P < 0.05).Patients with appendiceal tumors are more likely to have older onset age,longer course of disease,normal white blood cell count and elevated carcinoembryonic antigen level.There were statistically significant differences between the two groups in the external diameter of the appendix,wall thickness,whether the wall was accompanied by nodules,whether the cavity was accompanied by fecal calculus,whether the peripheral mesangium was thickened,and the shape of the appendix,including "onion skin" change,"sausage" change,and "spherical" expansion of the tip(P< 0.05).The external diameter and wall thickness of the appendix in patients with appendix tumor are larger than those in patients with appendicitis.The wall with nodules,intraluminal with fecal stones,and mesenteric thickening are more common.Among the changes in the shape of the appendix,"onion skin" like changes,"sausage" like changes,and "spherical" expansion of the tip were more common in tumor patients than in inflammatory patients;Multivariate logistic regression analysis showed that there were statistically significant differences in age,outer diameter,wall thickness,thickening of peripheral mesangium,"sausage" like changes,and "spherical" expansion of the tip(P<0.05).The index with higher clinical significance for the diagnosis of tumor patients is the external diameter of the appendix(AUC: 0.922),followed by the wall thickness(AUC:0.838).Conclusion: The sonograms and clinical manifestations of appendiceal tumors have certain characteristics.
