| ObjectiveBy researches on TCM treatment in therapeutic effect, length of stay and hospital costs of patients after appendectomy for complicated appendicitis, this project aims to find better treatment plan for patients’clinic treatment after appendectomy for complicated appendicitis and bring patients more economic benefits.Methods53patients with complicated appendicitis that meet the inclusion criteria are selected and divided into two groups as the TCM treatment group (28patients) and the control group (25patients) randomly regarding postoperative treatment. Conventional western medicine treatments, including fasting, anti-infection, symptomatic relief of pain and maintenance treatment of water and electrolyte balance, etc. are performed to the control group after operation. Based on the conventional western medicine treatment, Chinese medicine treatment is performed to the TCM treatment group, according to the prescription called old TCM doctor Zhao Bingnan’s appendix empirical prescription addition and subtraction:coix seed30g, magnol ia officinal is15g, beautiful sweetgum fruit15g, ricepaperplant pith10g, gorgon fruit20g, hyacinth bean20g, white muscardine silkworm15g, immature bitter orange10g, alpinia galanga5g, raw rhizoma atractylodis macrocephalae20g, orange peel15g and papaya15g. Herbs from the above prescription shall be decocted with500ml water to150ml and the decoction should be taken when it is warm. Patients in the TCM treatment group start to take their first dose of Chinese medicine6to12hours after the operation (with fasting but no prohibition of decoction), taking process of which has been divided into3times with warm decoction and3to4hours’ intervals; thereafter, the patients in the TCM treatment group take2times of warm decoction respectively in the morning and at night, a dose of Chinese medicine each day; and electrical acupuncture is performed to the patients at the same time, selecting the Zusanli acupoint and Shangjuxu point as its main acupoints and combining Tianshu point, Sanyinjiao point and Yinlingquan point, selecting the two sides of all of them; electrical acupuncture is performed for the first time6to12hours after the operation, with duration of20minutes each time and2times a day, until the patients have a bowel movement and can eat normally, then the patients stop taking of Chinese medicine decoction and electrical acupuncture. Discharge can be arranged for patients in both of the groups when their symptoms and signs disappear and their gastroenteric function, temperature and white blood cell return to normal. Clinical indicators (the first postoperative flatus, length of stay and hospital costs) of the patients are observed, statistical analysis of data of clinical indicators after appendectomy for complicated appendicitis are performed with SPSS16.0, all the data is expressed as mean+standard deviation (x±s) and results are processed by hypothesis testing(t test) regarding comparison of mean between the two groups, with statistical significance(P<0.05).ResultsComparisons of basic information like age, gender, pathology, etc. of the53patients between the two groups after operation are of comparability (P>0.05). Comparisons of the first postoperative flatus, length of stay and hospital costs between the two groups after operation are performed, with P <0.05. No complication of postoperative bleeding, appendix stump inflammation and stump fistula occurs in both of the two groups and there are4patients with incision infection (1in the TCM treatment group and3in the control group). Within comparison of the two groups, the TCM treatment group is obviously better than the control group in shortening of patients’first postoperative flatus, length of stay and reduction of their hospital costs.ConclusionThe therapeutic approach of combining Chinese medicine with electrical acupuncture to facilitate gastrointestinal peristalsis that adopted for patients after appendectomy for complicated appendicitis is with satisfactory outcomes; by comparisons between the TCM treatment group and the control group, it is found that the TCM treatment group is of more obvious curative effect, accelerating patients’recovery of gastroenteric function after operation, shortening the whole length of stay and reducing patients’hospital costs. This provides a new treatment thought for rapid recovery of patients after appendectomy for complicated appendicitis by clinical diagnosis and treatment and enables Chinese medicine to exert more extensive effects in diagnosis and treatment of modern diseases. |
