Background Acute appendicitis remains one of the most common gastrointestinal emergency and appendectomy is one of the most frequently performed surgical procedures because approximately 7%-9%of the population in Western countries experience appendicitis at some time in their lives?Since its description by Mc Burney in 1894 the open approach has become the gold standard surgical intervention for appendicitis,remaining virtually unchanged for 1 00 years.Along with the development of the laparoscopic technique,Laparoscopic appendectomy has become the gold standard for acute appendicitis in last years.Appendix stump closure is key point in laparoscopic appendectomy?Since the technology of Laparoscopic appendectomy was developed,the treatment methods of appendix stump closure are diffent.There has been no accepted conventional approach to appendix stump closure?Objective To compare the clinical effects and their advantages and disadvantages of hem-o-lok clip and ligation and pouch suture after embedding the application effect of laparoscopic appendectomy.Methods Retrospective analysis a total of 95 patients with acute Suppurative Appendicitis who underwent laparoscopic appendectomy in The Third Affiliated Hospital Of Xinxiang Medical University from July 2017 to June 2018 were chosen as the research objects,and they were divided into observation group and control group according to the grouping method of random number table,The observation group were given embeded with purse-string suture after mesenteriolum was cut off(n=65),While the control group were given the appendiceal stump directly to Hem-o-lok clipping(n=30).The operation time,amount of bleeding,hospital stay,appendix stump fistula,appendix stump inflammation,and complications between the two groups were observed and compared.Results There is no significant difference existed in appendix stump fistula[0% vs 3.3%,P =0.690],appendix stump inflammation[0% vs 0%]and hospital stay[(6.38±2.45)d vs.(7.93±2.98)d,P = 0.147],amount of bleeding [(12.92±6.43)ml vs(11.17±8.68)ml,P =0.272],and hospital stay[(6.38±2.45)d vs.(7.93±2.98)d,P = 0.147] between two groups.Hem-o-lok clipping group showed significantly shorter operation time than purse-string suture group[(73.83±22.15)min vs(108.54±34.97)min,P = 0.000].While the hospitalization expenses of the purse-string suture group was significantly less than that of the Hem-o-lok group [(8982.02±2172.76 yuan vs(10318.77±2596.09)yuan,P =0.010].Conclusions Although spending a little longer operation time,treatment of the appendicular stump using purse-string suture,compared with who were directly to Hem-o-lok clipping,hospitalization expenses can lesser.
