| Objective : To investigate the relationship between monocyte/high-density lipoprotein ratio(MHR)and cardiac structure and function in heart failure with preserved ejection fraction(HFp EF).Methods :Patients(n=213)who were continuously diagnosed with HFp EF in the geriatric department of First Hospital of Lanzhou University from January 2018 to December 2020 were selected as the HFp EF group,and healthy people(n=67)during the same period served as the control group.Taking the 75 th quantile of MHR level in the control group as the cut-off point,patients in the HFp EF group were divided into two subgroups,MHR ≤ 0.40(n=91)in Group I,and MHR>0.40(n=122)in Group II.Basic clinical data,laboratory examination results and MHR were collected,and cardiac structure and fraction were examed in all study subjects.Differences were compared between the HFp EF group and the control group,as well as MHR subgroups in the HFp EF group.Linear correlation and multiple linear regression were used to analyze the relationship between MHR and heart structure and function in HFp EF patients.Results :Compared with the control group,the levels of white blood cells(WBC),monocytes(MONO),neutrophils(NEUT)and MHR in HFp EF group increased,and the level of high-density lipoprotein cholesterol(HDL-C)decreased(P<0.05);(2)Subgroup analysis: compared with MHR group I,left ventricular end diastolic diameter(LVIDd),left ventricular end systolic diameter(LVIDs),interventricular septum end diastolic thickness(IVSTd),interventricular septum end systolic thickness(IVSTs),Epeak deceleration time(DT),right ventricular base diameter(RV-D1),right ventricular middle diameter(RV-D2),right ventricular base to apex length(RV-D3),inner diameter of the right ventricular outflow tract(above the aortic valve)(RVOT1),inner diameter of the short axis of the right atrium(RA-D1),inner diameter of the long axis of the right atrium(RA-D2),and the right atrium area(RAA)were increased in MHR group II(P<0.05);(3)Correlation analysis showed that MHR was positively correlated with LVIDd,left ventricular end diastolic volume(LV-EDV),left ventricular end systolic volume(LV-ESV),left atrial volume(LAV),RV-D1,RV-D2,RV-D3,RVOT1,RA-D1,RA-D2,and RAA(P<0.05);(4)Multiple linear regression analysis showed that MHR was independently correlated with systolic blood pressure,UA,LV-EDV,and RV-D3(P<0.05).Conclusions :In the HFp EF group,MHR increased,and the higher the MHR level,the larger LVIDd,LVIDs,IVSTd,IVSTs,DT,RV-D1,RV-D2,RV-D3,ROVT1,RA-D1,RA-D2,RAA cardiac remodeling;MHR is independently correlated with blood pressure,UA,LV-EDV,and RV-D3. |
