Objective:1.To translate the English version of the Fear of Complications Questionaire(FCQ)into a Chinese version.2.To evaluate the psychometric indicators of the Chinese version of the Fear of Diabetic Complications Questionnaire(FCQ).3.Using the Chinese version of the FCQ To investigate the current situation of the fear of complications in diabetic patients,and explore its influencing factors affecting the fear of complications in diabetic patients.Method:1.Sinicization of the questionnaire: Authorized by the original author of the questionnaire,the contextual relevance of the Fear of Diabetic Complications Questionnaire(FCQ)was quantitatively assessed using the Context Relevance Index(CRI),and the modified Brislin bidirectional translation model was used to evaluate the The Fear of Diabetes Complications Questionnaire(FCQ)was translated,and an expert committee was organized to conduct cultural adjustment and pre-investigation.The Chinese version of the Fear of Diabetes Complications Questionnaire(FCQ)was obtained after revision according to the results of the pre-investigation.2.Psychometric evaluation of the questionnaire: Using a cross-sectional survey design,325 diabetic patients from two hospitals in Yueyang City were selected as the research objects to evaluate the psychometric indicators of the Chinese version of the FCQ.The item total correlation method and the critical ratio method were used for item analysis;the internal consistency reliability Cronbach’s alpha coefficient and the test-retest reliability of 30 subjects at 2-week intervals were used to evaluate the reliability of the questionnaire;content validity,construct validity were used to evaluate the reliability of the questionnaire.The validity of the questionnaires was evaluated by the validity of the questionnaires,including the validity,convergent validity and criterion-related validity.Content validity was evaluated using the Content Validity Index(CVI),construct validity using exploratory factor analysis,and convergent validity using the Chinese version of the Hospital Anxiety and Depression Scale(Hospital Anxiety and Depression Scale).HADS)and the Chinese version of the Hypoglycemia Fear Survey II-Worry Scale(CHFSⅡ-WS)were used for correlation analysis,and the criterion-related validity was evaluated using the Chinese version of the Fear of Progression Questionnaire-Short Form(Fear of Progression Questionnaire-Short Form).,Fo P-Q-SF)as the calibration standard for correlation analysis.Data normality test,descriptive analysis,reliability analysis,correlation analysis and exploratory factor analysis were carried out using SPSS25.0 software.3.Preliminary application of the questionnaire: using SPSS25.0software to analyze the data,using descriptive analysis to obtain the current situation of diabetes patients’ fear of complications,using univariate analysis and multivariate analysis to explore the influencing factors of diabetes patients’ fear of complications.Result:1.The Chinese version of the questionnaire and the evaluation of psychometrics: The completion time of the Chinese version of the Diabetic Complications Fear Questionnaire ranges from 6 to 10 minutes,which is easy to understand and fill in.The scores of each item have a high correlation with the total score,and there is a significant difference in the scores of each item in the high group and the low group.The Cronbach’s alpha coefficient of the questionnaire was 0.954,and the test-retest reliability of the 30 subjects at a 2-week interval was 0.936(P<0.01).The item-level content validity index I-CVI of the Chinese version of the FCQ questionnaire ranged from 0.830 to 1.000,and the mean of all item-level I-CVIs,that is,the scale-level content validity index S-CVI/Ave,was 0.955.The scores of the Chinese version of the FCQ were significantly different from those of the Chinese version of the Hospital Anxiety Questionnaire(HADS anxiety,HADS-A),the Chinese version of the Hypoglycemia Fear-Worry Questionnaire(CHFSⅡ-WS)The correlation coefficients were 0.578,0.412 and 0.448(p<0.01).The correlation coefficient between the scores of the Chinese version of the FCQ and the scores of the Chinese version of the Fo P-Q-SF was 0.723(p<0.01).A total of 4 common factors were extracted by exploratory factor analysis,and there was no cross-loading,and the cumulative variance contribution rate accounted for 75.594%.2.Preliminary application of the questionnaire: In this study,the complication fear score of diabetic patients was 24.3±7.423 points,of which 76 patients had FCQ score ≥30 points,accounting for 23.4%.The top 3 items with the highest scores are:(10)Have you ever worried about diabetes affecting your future health(2.08±0.70)points;(1)I am afraid of long-term complications of diabetes(2.04±0.73)points;(12)When Do you worry about future health problems when your blood sugar is unstable(2.00±0.74).Univariate analysis showed that different age,education level,medical burden,disease duration and complications had significant influence on the fear of diabetic complications(P<0.05);multivariate logistic regression analysis showed that Sex(OR=0.502,95%CI: 0.289,0.873,P<0.05),BMI(OR=1.79,95%CI: 1.068,3.002,P<0.05),disease duration(OR=2.043,95%CI: 1.371,3.046,P<0.01)and the number of complications(OR=5.720,95%CI: 3.607,9.070,P<0.01)were independent influencing factors of Fo C.Conclusion:1.The translation process of the Chinese version of the Diabetic Complications Fear Questionnaire is scientific and rigorous,has good reliability and validity,is simple,convenient,and easy for patients to fill out.specificity assessment tool.2.Diabetic patients have a certain fear of complications of diabetes,and nearly 1/4 of diabetic patients are afraid of psychological dysfunction.Chinese patients are most afraid of the impact of diabetes on future health,long-term complications and the impact of blood sugar instability on health,and the fear scores of specific complications in the heart,eyes,feet,kidneys,etc.In nursing work,attention should be paid to the knowledge of diabetes patients on specific complications of diabetes,and a systematic and standardized health education program should be established to improve the cognitive level of diabetes on specific complications.3.Age,gender,BMI,educational level,medical burden,course of disease,and complications are related to the fear of diabetes complications.The course of disease and the number of complications are important factors affecting the fear of complications in diabetic patients.Fear of complications is associated with anxiety,depression,and fear of hypoglycemia.Nursing work should focus on the psychological state of diabetic patients with long course of disease and complications,assess the degree of fear of complications,care about the negative emotions of diabetic patients,carry out targeted psychological intervention,and teach patients to identify and effectively manage complications in a timely manner.Symptoms,improve patients’ awareness of complications and self-management,maintain blood sugar stability,thereby reducing the level of fear of complications. |
