The psychological problems in the course of cancer diagnosis and treatment have aroused widespread concern,such as depression,anxiety,fear of cancer recurrence and so on.At present,Fear of Cancer Recurrence(FCR)has become widespread psychological problems.Cancer patients found that FCR will bring different levels of psychological distress to patients,which is not conducive to the rehabilitation of patients.Recently,there has been a great deal of research on the FCR,but there are few studies on the assessment of FCR in China.On consulting a large number of literature.The study is based on a large amount of literature and research results of the corresponding fields at home and abroad.The introduction of compilation is suitable for the fear of cancer recurrence scale in our country.The scale was used to investigate the FCR in Shandong Province.The aim of this study is to provide a feasible and reliable evaluation tool for the assessment of FCR in Chinese cancer patients.In order to lay a theoretical foundation for further intervention of FCR.PART ?QUALITATIVE STUDY ON FEAR OF CANCER RECURRENCE IN BREAST CANCER PATIENTS ObjectiveThe study describes the psychological experience of breast cancer patients with FCR in China,puts forward the targeted clinical recommendations,and provides theoretical support and supplement for the further revision of the scale.MethodsA qualitative descriptive approach was choosen for this investigation.12 breast cancer patients were recruited from Affiliated Hospital of Taishan Medical University andcompleted an in-depth interview.Colaizzi's approach guided data analysis.ResultsThe theme and experience of FCR in breasts cancer patients.Theme 1: Emotional distress,mainly including:(1)thinking about FCR;(2)fear;(3)depression and anxiety;(4)unknown;(5)thinking about death;(6)the pressure of life.Theme 2: Trigger factors,mainly including:(1)worry about family;(2)media publicity(cancer related media information);(3)the medical appointment and treatment;(4)the physical symptoms.Theme 3: Coping strategies,mainly including:(1)positive response: seeking medical help;active treatment;be optimistic about life;(2)negative coping: cognitive avoidance;reduce social activity.ConclusionBreast cancer patients have different degrees of FCR and a variety of negative emotional experience,cancer patients will take different coping strategies.Clinical nurses should provide effective guidance methods to improve patients' negative psychological emotions.Part ?REVISION THE FEAR OF CANCER RECURRENCE INVENTORYObjectiveTo translate the Fear of Cancer Recurrence Inventory(FCRI)into Chinese and test it's reliability and validity.In order to provide a reliable and valid measure of FCR among cancer patients in China.Methods1.Translation and development: With the permission of Professor Simard S,the researcher has obtained the Fear of Cancer Recurrence Inventory use authorization.The research follows the scale reference application.Adopt the internationally used "Standardized forward-backward procedure-Expert Consultation-Cultural debugging" steps to compile the scale.Translated into the process of the formation of the FCRI of the draft A,draft B,draft C and draft D,after cultural adjustment,preliminary experiment to form the FCRI-CV draft which one iterm modified and the final total of 42 entries.2.Psychometric testing: A total of 140 cancer patients were recruited from three hospitals in Shandong province and were tested using the Chinese version of FCRI.Reliability and validity of the scale strictly tested.Thus accomplished the formation of Chinese version of Fear of cancer recurrence scale.Results1.The items and subscales of FCRI-CV: including 42 items and 7 subscales.Using Likert 5 count.And 13 items were scored inversely,total score range 0-168.The higher score,the higher of the FCR.2.Index of Content validity(CVI)was 0.841.Exploratory factor analysis showed that the factor loadings were above 0.3.The 7 common factors of precipitation were consistent with the original scale including triggers,severity,coping strategies,psychological distress,functioning impairments,insight,reassurance,explaining 58.33% of total variance.Criterion related validity indicate the dimensions of the FCRI-CV and subscales were positively correlated with Hospital Anxiety and Depression Scale and Impact of Events Revised.The Cronbach's alpha coefficient of the FCRI-CV which with good reliability and validity was 0.90,and the test-retest reliability was 0.71~0.86.ConclusionThe FCRI-CV has good reliability and validity,which can be used to evaluate the degree of FCR in patients with cancer.The future need to expand the sample size for further verification.Part ?PRELIMINARY CLINICAL APPLICATION OF CHINESE VERSION OF FEAR OF CANCER RECURREN CE INVENTORYObjectivePreliminary apply FCRI-CV,investigate the FCR in cancer patients and describe it's influencing factors.MethodsUsing FCRI-CV,Hospital Anxiety and Depression Scale and Impact of Events Revised scale to investigate the FCR of cancer population in our country,a questionnaire survey was conducted among 420 cancer patients in three hospitals from Shandong Province,to describe the present situation and analyze the influencing factors of FCR.ResultsThe investigation results show that the lowest score was 42 points of cancer patients,with the highest score of 146 points,the average score was(91.71±21.66).The total average score of FCR was 2.18±0.52,which was significantly different from that of the intermediate value of the item of 2(p<0.05).60.4% of cancer patients with moderate or above cancer recurrence were scored.The One-Way ANOVA and correlation analysis show that age,occupation,work status,education level,the awareness of the disease,primary caregiver,status of children,family income,the time of diagnosis,self-care ability,types of cancer,hospital anxiety and depression,impact of events were statistically significant,which are the influence factors of FCR.Multiple stepwise regression analysis showed that age,cancer types,depression,anxiety,events and children were predictors of FCR.ConclusionMore than half of cancer patients have different degrees of FCR.Age,occupation,work status,education level,the awareness of the disease,primary caregiver,status of children,family income,the time of diagnosis,self-care ability,cancer type to hospital depression and anxiety as well as impact of events are the factors of FCR.Age,cancer types,depression and anxiety,impact of events are the predictor of FCR.
