| Objective : To observe the efficacy of auricular acupressure combined with stilt-vein acupuncture treatment on spastic hemiplegia,and to explore its feasibility and effectiveness in reducing muscle tone of the limbs of stroke patients,in order to seek an effective rehabilitation treatment method for spastic hemiplegia after stroke in traditional medicine.Methods:The subjects of the study was derived from 72 patients who developed spastic hemiparesis after stroke in the Neuro Rehabilitation Department of Affiliated Rehabilitation Hospital of Fujian University of Chinese Medicine from November 2018 to January2022.Affiliated Rehabilitation Hospital of Fujian University of Chinese Medicine.These 72 patients were randomly divided into treatment and control groups by random number table,with 36 patients in each group.Patients in both groups were treated with basic therapy(including rehabilitation);the control group was treated with traditional acupuncture intervention(acupuncture to antagonize the muscle)on the basic therapy,and the treatment group was treated with auricular acupressure with stilt-vein acupuncture on the basic therapy.The spasticity of the hemiplegic limb was evaluated by the Modified Ashworth scale(MAS)and Clinical Spasticity Index(CSI)and the Fugl-Meyer Assessment scale(FMA)in the two groups at the time of enrollment and 4 weeks later.Motor function and Modified Barthel Index(MBI)scale to assess daily activities.Results:1.Baseline data:There were no statistically significant differences between the two groups in gender,age and course of disease(P>0.05).2.Modified Ashworth Scale(MAS)comparison: The clinical efficacy between treatment and control groups was significant(P<0.05).After both treatments,the spasticity was improved by 15,17,88.89%,7,16,and 55.56%,indicating that the treatment group was higher than that of the control group.3.Comparison of clinical spasticity index(CSI):There was no significant difference in the CSI score between the treatment group and the control group before treatment(P>0.05).The CSI scores before and after treatment were 12.17±1.48 and 9.72±1.43,respectively,and the difference was statistically significant(P<0.05).The CSI scores in the treatment group after treatment were significantly lower than those in the control group(P<0.05).4.Modified Barthel Index(MBI): There was no significant difference in the MBI score between the treatment group and the control group before and after treatment(P>0.05).The MBI scores before and after treatment were 47.36±10.11 and 58.33±10.14,respectively,and the difference was statistically significant(P<0.05).The MBI score of the treatment group after treatment was significantly higher than that of the control group(P<0.05).5.Simple Fugl-Meyer Scale(FMA):There was no significant difference in the FMA scores between the treatment group and the control group before and after treatment(P>0.05).The FMA scores of the group before and after treatment were 41.28±5.62 and 56.22±4.97,respectively,and the difference was statistically significant(P<0.05).The FMA score of the treatment group after treatment was significantly higher than that of the control group(P<0.05).Conclusions:Compared with the traditional antagonistic muscle acupuncture treatment,auricular acupressure with stilt-vein acupuncture has better efficacy in post-stroke spastic hemiplegia,which can further reduce the severity of limb spasticity,improve the level of motor function of patients,earlier and better achieve self-care and return to society as much as possible.This combined treatment protocol can be used as a routine treatment for post-stroke spastic paralysis. |
