Validity And Application Of Union Physio-Psycho-Social Assessment Questionnaire In Outpatients From General Hospital

Objective To test the validity of Union Physio-Psycho-Social Assessment Questionnaire(UPPSAQ-70)in screening anxiety and depression in outpatients from general hospital.The UPPSAQ-70 was used to describe the symptom clusters and somatic symptoms in outpatients with anxiety and depression in general hospitals.Methods Patients from Peking Union Medical College Hospital from June 2020 to November 2021 were recruited.They were assessed with UPPSAQ-70,Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS).Clinical diagnosis,made by outpatient doctors according to the diagnostic criteria of ICD-10,is considering as the"gold standard".The validity of the UPPSAQ-70 was tested by correlation test,drawing the receiver operating charactristic curve(ROC),and comparing the differences between the anxiety and depression group and the healthy control group.The characteristics of symptom clusters and somatic symptoms of patients with anxiety disorders,depressive disorders,and mixed anxiety and depression in general hospital outpatients were described by comparing differences between groups and drawing profile.Result(1)The UPPSAQ-70 total score showed significant correlations with SAS(r=0.843,p<0.01)and SDS(r=0.852,p<0.01).The area under the curve(AUC)of the UPPSAQ70 was 0.875(P<0.001),the cut-off value was 65 points with the sensitivity 83.2%and the specificity 75.5%.The AUC of the SAS and SDS was 0.848 and 0.808 respectively(both P<0.001).Except for the factor of sexual function and feeling,the total score,the scores of three dimensions and other factors of UPPSAQ-70 in the anxiety and depression group were higher than those in the control group(P<0.01).(2)The profile of anxiety disorders,depressive disorders,and mixed anxiety and depression were similar,but the severity was different.The positive rate of somatic symptoms in the patient groups was higher than that in the healthy control group,but there was no significant difference in Q1,Q6 and Q17 items.Conclusion UPPSAQ-70 can be used to screen anxiety and depression in outpatients in general hospitals with good validity and high sensitivity.According to the characteristics of patients in general hospitals,UPPSAQ-70 uses a single scale for multi-dimensional evaluation,which provides a powerful tool for psychometric in general hospitals.The distribution of symptoms of patients with depressive disorders,anxiety disorders and mixed anxious-depressive disorders in the outpatients of general hospital is similar,and they all have extensive and prominent somatic symptoms.
