Abstract/Summary:
Objective: Exploring the therapeutic effect of Qingfei Huatan Tang on patients with acute exacerbation of chronic obstructive pulmonary disease(phlegm heat stagnation lung syndrome)and its impact on MUC5 AC from the perspective of high secretion of airway mucus. Methods: Patients with acute exacerbation of chronic obstructive pulmonary disease(COPD)who meet the inclusion criteria(phlegm heat stagnation lung syndrome)were randomly divided into a control group and a treatment group, with 30 cases in each group. The control group received routine Western medicine treatment(controlled oxygen inhalation, anti infection, expectorant, bronchiectasis, etc.),while the treatment group received oral Qingfei Huatan Tang twice a day,100 ml each time,in addition to the control group. Both groups were treated for 10 days.Observation indicators:(1)TCM symptom score, sputum character score, arterial blood gas analysis(Pa O2,Pa CO2),WBC,N%,CRP value changes before and after treatment;(2)The Elisa method was used to detect the levels of NE in the patient’s serum and MUC5 AC in sputum before and after treatment.Results:(1)Clinical efficacy comparison: The total effective rate of the treatment group was 96.67%,while the total effective rate of the control group was 86.21%.,The treatment group was superior to the control group(P<0.05).(2)Comparison of total points of traditional Chinese medicine symptoms: There was no statistically significant difference in the total points of traditional Chinese medicine symptoms before treatment between the treatment group and the control group(P>0.05),and the two groups were comparable; After treatment, the total symptom score of both groups of patients significantly decreased compared to before treatment(P<0.01),and the total symptom score of the treatment group was significantly lower than the control group after treatment, with a highly statistically significant difference(P<0.01).(3)Comparison of single symptom scores in traditional Chinese medicine: Before treatment, there was no statistically significant difference in the scores of cough, expectoration, wheezing, fever, chest tightness, and fatigue between the two groups of patients(P>0.05),and the two groups were comparable; After treatment, the scores of cough, phlegm, wheezing, fever, chest tightness, and fatigue in both groups were significantly reduced compared to before treatment(P<0.01);After treatment, the comparison between the two groups showed that the treatment group was better than the control group in improving cough and sputum symptoms(P<0.05),and significantly better than the control group in improving fatigue symptoms(P<0.01);There was no significant difference between the two groups in improving symptoms of fever, wheezing, and chest tightness(P>0.05).(4)Comparison of sputum trait scores: Before treatment, there was no statistically significant difference in sputum trait scores between the two groups(P>0.05),indicating comparability; After treatment, the sputum trait scores(sputum color, sputum viscosity, and 24-hour sputum volume)of both groups were significantly reduced compared to before treatment(P<0.01);After treatment, the comparison between the two groups showed that the treatment group was better than the control group in reducing sputum volume(P<0.05),and significantly better than the control group in improving sputum color and viscosity(P<0.01).(5)Comparison of blood gas analysis indicators:Before treatment, there was no statistically significant difference in Pa O2 and Pa CO2 values between the two groups(P>0.05),indicating comparability; After treatment, the Pa O2 index in both groups significantly increased compared to before treatment, while the Pa CO2 index significantly decreased compared to before treatment(P<0.01);After treatment, the comparison between the two groups showed that the treatment group was superior to the control group in improving Pa O2 and Pa CO2 indicators(P<0.05).(6)Comparison of laboratory indicators: Before treatment, there was no statistically significant difference in WBC,N%,CRP,NE,and MUC5 AC values between the two groups(P>0.05),indicating comparability; After treatment, the WBC,N%,CRP,NE,and MUC5 AC values of both groups were significantly reduced compared to before treatment(P<0.01);After treatment, compared between the treatment group and the control group, the treatment group showed a significant improvement in reducing NE and MUC5 AC levels compared to the control group(P<0.01),and a better reduction in N% values compared to the control group(P<0.05);There was no significant difference in reducing WBC and CRP values between the two groups after treatment(P>0.05).(7)Safety: According to the monitoring results of safety indicators, there were no significant toxic side effects or adverse reactions.Conclusion(s):(1)Qingfei Huatan Tang can significantly improve the clinical symptoms and respiratory function of AECOPD(phlegm heat stagnation lung syndrome)patients;(2)Qingfei Huatan Tang can reduce the levels of NE in serum and MUC5 AC in sputum of AECOPD(phlegm heat stagnation lung syndrome)patients, thereby inhibiting airway mucus hypersecretion.
