| Objective: Femoral neck fracture is one of the most common fracture types in clinic,and its incidence rate accounts for about 3.58% of the total fracture incidence.Elderly patients with femoral neck fracture after activity disorders,long-term bed will lead to a series of complications,serious threats to the health of the elderly,reduce the quality of life of patients and even life-threatening,so once diagnosed must take the correct treatment measures active treatment,in order to reduce fracture complications,improve the quality of life of patients,reduce medical expenditure.At present,experts at home and abroad agree that surgery should be the preferred treatment for femoral neck fracture in the elderly,and the main surgical methods include internal fixation and hip replacement.Because the elderly are associated with multiple underlying diseases and poor bone conditions,hip replacement has become an important surgical method for the treatment of femoral neck fracture in the elderly because it can be used for early ground movement and less risk of internal fixation.The application of traditional Chinese medicine in the treatment of femoral neck fracture in the elderly can reduce postoperative bleeding,reduce the risk of infection,regulate the biochemical indexes of bone metabolism,improve the patient’s systemic condition and so on,which has a high clinical promotion value.The purpose of this project is to study the short-term efficacy of Bu Shen Yang Xue ointment prescription in the treatment of elderly patients with liver and kidney deficiency femoral neck fracture after hip replacement,explore new methods of integrated Chinese and western medicine in the treatment of elderly patients with femoral neck fracture,and further study and understand the academic thought of traditional medicine of "treating both internal and external".Methods: The 60 cases meeting the inclusion criteria were divided into two groups by random number table method,namely the treatment group and the control group,with 30 cases in each group.Patients in the control group and the treatment group were treated with hemostasis,detumescence,anti-infection,analgesia,anticoagulation and other conventional treatments after artificial hip replacement,and patients in the control group were orally treated with Bu Shen Yang Xue ointment prescription.After that,the remission of postoperative symptoms,incidence of complications and postoperative recovery of the two groups were evaluated.The main observation indicators of this study include visual simulation score,inflammation index,blood loss index,coagulation function,blood stasis state,traditional Chinese medicine syndrome score,clinical efficacy,etc.The data were statistically analyzed by SPSS 26.0 statistical software,and test methods such as parametric test,non-parametric test and Chi-square test were selected according to the data characteristics.P < 0.05 was considered to be statistically significant.Results:(1)Comparison of postoperative VAS scores between the control group and the treatment group: After treatment,VAS scores of the control group and the treatment group were lower than before,and the VAS scores of the control group were lower than those of the treatment group,and the difference was statistically significant.(2)Comparison of postoperative inflammatory indicators between the control group and the treatment group: After treatment,the white blood cell count and serum C-reactive protein and other inflammatory indicators in the control group and the treatment group were lower than before,and the white blood cell count and serum C-reactive protein in the control group were significantly lower than those in the treatment group,the difference was statistically significant.(3)Comparison of postoperative blood loss indexes between the control group and the treatment group: After treatment,the hemoglobin and hematocrit of the control group and the treatment group were increased,and the hemoglobin and hematocrit of the control group were significantly higher than that of the treatment group,the difference was statistically significant.(4)Comparison of thrombin function between the control group and the treatment group: After treatment,the prothrombin time and activated partial thrombin time of the control group and the treatment group were shorter than before,the control group was significantly shorter than the treatment group,the difference was statistically significant.(5)Comparison of blood stasis state between control group and treatment group: After treatment,D-dimer and fibrinogen contents in control group and treatment group were lower than before,while D-dimer and fibrinogen contents in control group were significantly lower than those in treatment group,and the difference was statistically significant.(6)Comparison of traditional Chinese medicine syndrome scores between the control group and the treatment group: After treatment,traditional Chinese medicine syndrome scores of the control group and the treatment group were shorter than before,and traditional Chinese medicine syndrome scores of the control group were significantly lower than those of the treatment group,the difference was statistically significant.(7)Comparison of clinical efficacy between the control group and the treatment group: the overall effective rate of the control group and the treatment group was 100%,the difference was not statistically significant.Conclusion:(1)The application of Bu Shen Yang Xue ointment in elderly patients with femoral neck fracture of liver and kidney deficiency after hip replacement can relieve postoperative pain,reduce inflammatory indicators,promote the recovery of hemoglobin,reduce postoperative invisible blood loss,improve coagulation function,relieve blood hypercoagulation and stasis,and improve traditional Chinese medicine syndromes.It has definite curative effect,safe and reliable,and has clinical application value.(2)Hip replacement is an effective surgical method for the treatment of femoral neck fracture in the elderly,which can quickly relieve the clinical symptoms of femoral neck fracture,reduce the pain of patients,and has satisfactory clinical efficacy. |
