Effect Of Huayu Qushi Decotion On The Expression Of Pain Factors In Peripheral Blood Of Patients With Early And Middle Stage KOA And Evaluation Of Its Therapeutic Effect

Objective: This study mainly uses the Huayu Qushi Decotion to treat Knee Osteoarthritis(KOA)patients with phlegm dampness stagnation type.By observing the visual analogue scale(VAS score),Xi’an University of Ontario and Mc Master University Osteoarthritis Index(WOMAC score),traditional Chinese medicine symptom quantification score,and detecting pain factors such as serum serotonin(5-HT)and prostaglandin E2(PGE2)The expression levels of nerve growth factor(NGF)and Calcitonin related peptide(CGRP)were compared with those in the western medicine group,and then the effect of Huayu Qushi Recipe on serum pain factors in patients with phlegm dampness stasis type KOA was discussed.Methods: ccording to the case selection criteria of this study,42 patients with phlegm dampness stasis type KOA diagnosed in the orthopedic outpatient department of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine were selected.They were randomly divided into two groups according to the random number table method,namely the treatment group with 21 cases and the control group with 21 cases;The treatment group was given an oral Decotion for resolving blood stasis and removing dampness,while the control group was given oral iricoxib tablets.All participants voluntarily signed relevant informed consent forms.Based on the comprehensive efficacy indicators observed before and after 2 weeks of treatment,including VAS,WOMAC,and TCM symptom quantification scores,as well as changes in serum pain factors(5-HT,PGE2,NGF,CGRP)levels,and compare them.All collected experimental data will be processed and statistically analyzed using SPSS 26.0 application software.Results:In this study,3 cases were dropped out,and a total of 39 patients were included in the statistical analysis,including 20 cases in the treatment group and 19 cases in the control group.(1)Before treatment: By comparing the baseline data(gender,age,course of disease,unilateral(bilateral),knee K-L grade),clinical efficacy index VAS,WOMAC,TCM symptom quantification and other scores and the expression level of pain factors 5-HT,PGE2,NGF and CGRP between the two groups,it was found that there was no difference between the two groups(P>0.05),and the two were comparable.(2)Intra-group comparison: After treatment,the VAS,WOMAC score,total score of TCM symptoms and the expression level of 5-HT,PGE2,NGF and CGRP in the two groups decreased compared with that before treatment,with statistical difference(P<0.05);However,among the symptoms of KOA,except for the daily activities,there was no difference(P>0.05),the other symptoms were significant(P<0.05),indicating that both Huayu Qushi Decoction and iricoxib could improve the knee joint pain symptoms and restore the knee joint activity of patients.(3)Comparison between groups: After treatment,the WOMAC score,total score of TCM symptoms,5-HT,PGE2,NGF and CGRP in the treatment group were significantly lower than those in the control group(P<0.05);However,there was no significant difference in VAS score between the two groups(P>0.05);It is believed that Huayu Qushi Recipe has advantages over iricoxib in improving the pain of patients and reducing the expression level of pain factors in serum,but there is no significant difference between the two in VAS score.In the comparison of KOA symptom scores,except for the difference between the two groups of knee joint swelling(P<0.01),there was no significant difference between the two groups of other symptoms(P>0.05),indicating that the effect of Huayu Qushi Decoction on improving knee joint swelling of patients was better than that of iricoxib.(4)Comparison of clinical efficacy: After the course of treatment,the total effective rate of the Huayu Qushi Decotion group was 90%,while the total effective rate of the Irecoxib group was 84.2%.The difference in total effective rate between the two groups was statistically significant(P=0.021<0.05);In summary,both the Huayu Qushi Decotion group and the Irecoxib group can improve the clinical symptoms and alleviate pain in KOA patients,but the overall treatment effect of Huayu Qushi Decotion is better than that of Irecoxib.(5)Safety evaluation: During the whole treatment process,all patients did not have any adverse reactions.Conclusion(s):(1)Huayu Qushi Decotion can effectively relieve the pain and swelling of knee joint of KOA patients with phlegm,dampness and stasis in the early and middle stages,and improve the range of motion of knee joint.(2)The Huayu Qushi Decotion can effectively reduce the expression levels of 5-HT,PGE2,NGF,and CGRP in the serum of KOA patients,which may be one of the effective mechanisms for treating joint pain in early and middle stage KOA patients.(3)In the process of this study,there were no adverse events observed in the Huayu Qushi Decotion and Irecxib group,confirming their safety and reliability,which is worthy of promotion and application.
