Objective: Double balloon is one of the main means of promoting cervical maturity and inducing labor in full-term pregnancy.The maximum water injection of internal and external balloon is 80 ml and 80 ml,but there is no unified standard for the water injection of double balloon in clinical use,and the effect of inducing labor is also very different.In this study,the influence of different internal and external balloon injection water on the total progress of labor and pregnancy outcome of puerpera was compared and analyzed,in order to obtain reasonable water injection volume of double balloon,and to provide certain reference basis for promoting cervical maturation and inducing labor in clinical primipara.Methods: From July 2019 to September 2021,638 hospitalized delivery patients in the obstetrics Department of Qingdao Municipal Hospital were selected,all of whom were primipara,full-term,with immature cervix and indications of labor induction.Double balloon was used to promote cervical maturation combined with artificial rupture of membrane and uterine hormone to induce labor.They were divided into four groups according to the different injection volume of internal and external balloon,which were 80/80 ml group(120 cases),80/60 ml group(200 cases),80/40 ml group(68 cases)and 60/40 ml group(250 cases).The statistical method was SPSS26.0,the count data were represented by example(%),and the Chi-square test was used for comparison between groups.Normal test was performed for all measurement data.Data subject to normal distribution were expressed as mean ± standard deviation;one-way analysis of variance was used among multiple groups;LSD test was used for pair comparison between groups;Wilcoxon rank sum test was used for data not subject to normal distribution.We analyzed the age,height,weight,body mass index,gestational age at delivery,composition of indications for inducing labor,effective rate of cervical maturation promotion,cervical score before and after balloon placement,cesarean section rate,vaginal delivery rate,indications of cesarean section,lateral perineal resection rate,incidence of acute labor,labor duration,postpartum blood loss,umbilical cord prolapse rate,postpartum bleeding rate,neonatal asphyxia rate,neonatal weight and length other indicators of the four groups of women at delivery.Results: 1.There were no significant differences in age,height,weight before delivery,body mass index(BMI),gestational weeks at labor induction,composition of indications for inducing labor,neonatal length and birth weight among the four groups(P > 0.05).2.There were significant differences in the effective rate of cervical maturation promotion among the four groups(P < 0.05),and the effective rate of cervical maturation promotion was higher in 80/60 ml group(95.50%)and 80/80 ml group(95.00%).The cervical score of the four groups was improved by 80/80 ml group(3.11±1.20)points,80/60 ml group(3.22±1.30)points,80/40 ml group(2.44±1.18)points,60/40 ml group(2.78±1.29)points,and there were significant differences among the four groups(P < 0.05).Among them,80/60 ml group and 80/80 ml group improved the score more.3.There were statistically significant differences in cesarean section rate among the four groups(P < 0.05),among which the 80/60 ml group(21.50%)Cesarean section rate is the lowest;The second stage of labor was significantly different among the four groups(P < 0.05),and the 80/80 ml group was significantly shorter than the other three groups.There was no significant difference in the first stage of labor,the third stage of labor and the total stage of labor among the four groups(P > 0.05).There were no significant differences in postpartum hemorrhage,postpartum hemorrhage incidence and umbilical cord prolapse rate among the four groups(P > 0.05).4.There were significant differences in the lateral perineal resection rate among the four groups(P < 0.05),and the 80/60 ml group(32.48%)and 80/40 ml group(26.92%)were significantly lower than those in 80/80 ml group(50.63%)and 60/40 m group(42.35%).There was no difference between 80/60 ml group and 80/40 ml group(P > 0.05).5.There were significant differences in the incidence of acute labor among the four groups(P < 0.05),which were 80/80 ml group(12.66%),80/60 ml group(8.92%),60/40 ml group(4.46%),80/40 ml group(3.85%),and the lateral resection rate of the other three groups was significantly lower than that of the 80/80 ml group.There was no significant difference among the other three groups(P > 0.05).Conclusion: Both internal and external balloon injection with different water volume can effectively improve cervical score and promote cervical maturation.In particular,the 80/60 ml group has a good effect on promoting cervical maturation,better reducing the rate of cesarean section,shortening the labor process and reducing complications.It is recommended to use 80/60 ml double balloon in clinic to promote cervical maturation and induce labor in first-time mothers during term pregnancy.
