Clinical Observation On Yiqitongmai Decoction In The Treatment Of Chronic Coronary Syndrome Complicated With Heart Failure With Preserved Ejection Fraction

Objective: To explore the clinical effect of Yiqi Tongmai decoction on chronic coronary syndrome(CCS)combined with heart failure with ejection fraction retention(HFpEF)with qi deficiency and blood stasis,and to assess its safety.Methods: From November 2021 to November 2022,66 patients diagnosed as CCS with HFpEF were selected from the outpatient department and sickrooms of Cardiovascular Department of the Affiliated Hospital of Shandong University of TCM.Then divided them into test group(TG)and control group(CG),with 33 cases in each group.TG was given Yiqi Tongmai decoction combined with western medicine,while CG was given western medicine.8 weeks later,the TCM syndrome score,TCM syndrome curative effect,6-minute walking test,NYHA garding,NT-proBNP,e/e’ rate,Minnesota Heart Failure Quality of Life Score,Seattle Angina Scale and nitroglycerin stopping rate were observed before and after treatment,and the curative effect of Yiqi Tongmai Decoction on patients with chronic coronary syndrome complicated with heart failure with preserved ejection fraction was explored,and the adverse reactions during the treatment were observed and recorded,and the safety was estimated.Results: According to the criteria of elimination,shedding and termination,63 patients were actually included,including 31 patients in CG and 32 patients in TG.1.TCM syndrome score: Comparison after 8 weeks,the TCM syndrome score of CG and TG decreased(P < 0.05);The curative effect of TG was better than that of CG(P <0.05).2.Curative effect of TCM syndrome: The total effective rate of TCM syndrome in TG was 81.25%,while that in CG was 54.84%,and the curative effect of TCM syndrome in TG was better than that in CG(P < 0.05).3.6-minute walking test: Comparison after 8 weeks,the 6-minute walking test distance of patients in CG and TG was improved(P < 0.05);The curative effect of TG was better than that of CG(P < 0.05).4.The curative effect of NYHA garding: The total effective rate of TG was 84.38%,and that of CG was 58.06%.The curative effect of traditional Chinese medicine on cardiac function in TG was better than that in CG(P < 0.05).5.NT-proBNP: Comparison after 8 weeks,the serum NT-proBNP levels in CG and TG decreased(P < 0.05);The curative effect of TG was better than that of CG(P < 0.05).6.e/e’ rate: Comparison after 8 weeks,the e/e’ rate in CG and TG were not improved(P > 0.05);TG and CG have similar curative effects.(P>0.05).7.Minnesota quality of life score of heart failure: Comparison after 8 weeks,the Minnesota quality of life score of heart failure in CG and TG decreased(P < 0.05);The curative effect of TG was better than that of CG(P < 0.05).8.Seattle Angina Scale: Comparison after 8 weeks,the scores of PL,AS,AF,TS,DS in CG and TG were all improved(P < 0.05),and the scores of PL,AS,AF and TS were all improved.TG and CG have similar curative effects.(P>0.05).9.Nitroglycerin stopping rate: The total effective rate of cardiac function in TG was84.38%,and that in CG was 61.29%.The curative effect of cardiac function in TG was better than that in CG(P < 0.05).Conclusion: Yiqi Tongmai decoction combined with routine western medicine can effectively improve the TCM syndrome integral of CCS with HFpEF of Qi deficiency and blood stasis type,increase the distance of 6-minute walking test,reduce NT-proBNP,reduce the number of angina pectoris attacks,improve the clinical symptoms and quality of life of patients,and have good safety.
