| Objective This study takes the clinical nursing path as the framework to construct a perioperative refined nursing scheme for cardiac permanent pacemaker implantation,and conducts an empirical study on it,in order to optimize the routine nursing plan,reduce the incidence of common postoperative discomfort in patients undergoing permanent pacemaker implantation,promote the recovery of patients,and provide a reference for the perioperative refined nursing of patients undergoing permanent pacemaker implantation.Methods Part I: Construction of perioperative refined nursing plan for permanent pacemaker implantation of the heart.Retrieve guidelines,expert consensus,original literature,systematic evaluations,etc.in the field of cardiac pacemakers,obtain the best evidence based on quality evaluation standards,analyze and extract the included literature materials,and after group discussion and combined with clinical experience,integrate refined nursing processes and measures into the clinical nursing pathway,and preliminarily form a refined perioperative care plan for permanent cardiac pacemakers.Through the Delphi Expert Consultation Method,15 experts who met the inclusion criteria were consulted,and the first draft was refined and revised to form the final draft.Part II: Clinical empirical studies.Using convenience sampling method,patients who planned to undergo permanent cardiac pacemaker implantation surgery at the Cardiovascular Department of the Third Class A Hospital in Qinhuangdao City from April 2021 to May 2022 were selected and numbered in the order of admission.The patients were divided into a control group(routine nursing)and an observation group(refined nursing)using random number table method,with 46 patients in each group.The basic preoperative information of two groups of patients is as follows: χ2 or t-test;Counting data is expressed in percentage(%),and inter group comparisons are made using χ2 Inspection;Measure the data using t-test.The incidence of common postoperative malaise symptoms was compared between the two groups,the Kolcaba’s General comfort questionnaire at 48 hours after surgery,the Activity of Daily Living(ADL)score at 72 hours after surgery,and the postoperative recovery quality score.Results Comparison of the incidence of common postoperative uncomfortable symptoms between the two groups,including the incidence of low back pain,stiffness of limbs,abdominal distention and difficulty in defecation,there was a statistically significant difference(P<0.05),the incidence of urinary retention,there was no statistically significant difference(P>0.05),and there was a statistically significant difference(P<0.05)in the score of Kolcaba’s General comfort questionnaire 48 hours after surgery,There was a statistically significant difference(P<0.05)between the 72 hour postoperative activity of daily living(ADL)score and the postoperative recovery quality score.Conclusions The perioperative refined nursing program constructed by this study can effectively reduce the incidence of common postoperative discomfort symptoms in patients,improve the comfort of patients during bed rest,reduce patients’ pain,shorten the recovery time of self-care ability,and improve the quality of surgical recovery. |
